When asked about whether it’s open to all DoD members or only active duty/veterans, my coworker said that that the description seemed inclusive.
The eligibility reads:

| Hello everyone, I have an Xpole Xpert Pro Pole (try saying that 10x fast) and I noticed with the new season that some nails are coming out of my plasterboard ceiling. I used a stud finder to put it up and I am 99,99% sure it is under a joist. Now in the XPERT manual they mention that this can happen, so I am trying to not get too freaked out. (You can see the little pins in the ceiling). But I am wondering, can I just hammer them back in? Or should I just leave like this? I am not moving my pole anytime soon. It is a rental, but it's okay because there is actually one other random spot on the ceiling (not close to the pole at all) where I saw one nail coming out too. It's an old house in europe lol. Anyone else that had this before and what is the best way to handle it? And if I have to hammer them in again, do I need to remove the pole or can I just keep it up? Just got back from 10 days in Japan and wanted to type up a trip report to hopefully help others that are trying to plan a trip!
Our trip was 5/16 - 5/28 including travel days. This was my 2nd time in Japan (first time was study abroad for 5 weeks in May/June 2016), but it was my mom & sister's first time, so the goal was to include as many must-see activities as we possibly could in the short time we were there. Our days were packed to the brim however all 3 of us are fairly active and healthy so despite being absolutely exhausted by the end of each day, in retrospect we wouldn't have changed a thing in our itinerary.
That being said, our itinerary would be much too busy for the average person's first trip to Japan so keep that in mind if you use this information to plan your own trip. For example, we usually only had time to sit down and eat for one meal per day and had to eat on the go for the other meals in order to have enough time to go to all the places we wanted to go to. There was very little downtime in our trip.
Tourist Tips
- Luggage
- We each traveled with 2 checked luggage since they were included in our flight. I had one full sized suitcase and one slightly smaller suitcase which both only had one side filled when I left the US. The plan was to fill the other half of the suitcases with souvenirs to bring home. Last time I went to Japan, it cost me ~$200 to ship two large boxes of souvenirs back home, so despite having to bring a lot of luggage this saved me a significant amount of money and I will be doing this for future trips as well.
- Within our luggage we also packed a duffel bag to hold all our purchases on heavy shopping days and a daily backpack to hold things like our trash bag, hand towel, hand sanitizewet wipes, body wipes, small purchases, etc. My carry-on was a 40L backpack that I took with us when we stayed overnight in Kyoto.
- Money
- I brought $400 cash to exchange at the airport. I had gotten a Charles Schwab debit card before leaving which has no foreign transaction/ATM fees so I planned to use my card for whatever purchases I could then use cash for everything else. I ended up withdrawing another ¥20000 which was just enough cash to last me the rest of our trip.
- Walking
- Last time I went to Japan I survived wearing flat gladiator sandals basically the whole trip, but this did NOT work this time around for whatever reason (I guess we walked a lot more this time?). BRING COMFORTABLE SHOES TO WALK IN. I ended up having to buy sneakers on day 2 because I stupidly didn't think I would need them. I was more concerned about looking cute for pictures (ha) but I quickly realized that all the girls in Japan wear sneakers with their cute skirts and dresses, so I blended right in. I would also recommend bringing a second pair of shoes with you each day (in your backpack) to swap into if the shoes you are wearing start giving you blisters.
- Masking
- We saw a lot of people not wearing masks, but the majority of people still wear masks. I would say the ratio of people wearing masks to not wearing masks was about 80:20.
- Trash
- As many have mentioned on this subreddit, due to the lack of public trash bins I would highly recommend bringing a reusable trash bag to store your trash while you're out and about until you can find a bin. We found these cool black fabric ones with a waterproof liner on the inside that you could rinse out when it got dirty and could clip to the outside of your backpack so it wasn’t with your other belongings.
- Restrooms
- Due to the lack of hand dryers/paper towels in bathrooms we also brought hand towels as recommended, but found ourselves just air drying our hands way more often than using the hand towels as it was often more cumbersome to remove our backpacks and fish out our hand towels to use.
Accommodations
- Sakura Hotel Jimbocho in Tokyo
- I stayed in this hostel when I studied abroad in college in 2016, so it holds a special place in my heart. You can get a single room here for cheap, and although the rooms are extremely small, I don’t travel to Japan to spend time in my hotel so when looking for accommodations price is my #1 factor, not comfort. Toilets and showers are shared in this hostel and the rooms are set up coming off of a square hallway with sinks/restroom facilities in the center. The facilities don’t appear to be very modern and pretty, but the water pressure of the showers is the BEST. They have other room types as well, like bunk-beds, double rooms, etc. They have a 24 hour cafe downstairs and breakfast is included in your stay. All their staff speak English & Japanese (and other languages as well). They host some cool events like language exchanges and walking/jogging groups if you are more of a social person. I really enjoy the location of this hostel. You are within walking distance from the Imperial Palace and it still feels like you are in the city without it being so loud and busy like if you were to stay in Shibuya, etc. They do have other locations if you’re looking for somewhere with more nightlife.
- The Millennials Hostel in Kyoto
- This is a really modern looking hostel in Kyoto, 2 streets over from Pontocho. The facilities are gorgeous and look brand new. They are set up similar to a capsule style hotel with a bunch of rooms coming off a hallway. Your bed takes up your entire space, and you have about 18 inches at the foot of your bed to stand. You pull an opaque privacy screen down to make your room private - there is no door. You can control your bed’s incline and lights through an iPhone in your room. There is storage space under your bed and on a shelf above your bed. Restrooms are shared but the showers are in one room and toilets in another. There are a lot of shared common spaces including a full stocked kitchen you can cook your meals in, but we literally only stayed here to sleep so we didn’t explore those spaces. I was worried about other guests being noisy but that wasn’t the case - everyone was super respectful. The price and location was great so I would definitely come back here.
Daily Itinerary
Day 1 & 2: Travel - We flew out of Chicago O'Hare International Airport. Our direct flight to Narita was around $1600 after taxes, fees, and flight insurance. I had one layover the last time I flew to Japan and in the future I will only be buying a direct flight as I found this to be way easier than having a layover.
- Once we arrived in Narita we turned on our e-sim. We chose Ubigi after reading many positive reviews on this subreddit and I highly recommend them. We had some issues getting iMessage to work despite following Ubigi's instructions to a T, so we ended up just using WhatsApp to message during the trip, however, iMessage did start working later on.
- We decided against picking up our JR passes & seat reservations at the airport because the line was long and we didn't need them for another 5 days.
- Going through customs/immigration and grabbing our luggage only took about 20-30 minutes. There were many staff to guide you and we found the whole process to be really smooth.
- We exchanged our currency and went downstairs to buy our tickets for Narita Express at a ticket vending machine. We got round trip tickets for ¥3460 (keep in mind that the return ticket needs to be used within 14 days). We had purchased our Suica before leaving and put them in our iPhone wallets and they came with a balance of ¥2000 so we didn't load money onto our cards until the following day.
- We took the Narita Express to Tokyo Station then took a cab to our hostel in Jimbocho. We each traveled with 2 checked luggage, so we found the short and inexpensive taxi ride to be a better option than lugging 6 giant suitcases on local trains to get to our hostel.
- We checked into our hostel. The total price per person for 11 nights was ¥49500 (about $380).
Day 3: Imperial Palace/Ueno - We woke up and walked to the Imperial Palace/East Gardens. We got there right when they opened and spent about 90 minutes there. We walked back to Jimbocho to go to my favorite tempura place in Jimbocho. This place has gotten quite famous since I was last there in 2016. We were first in line at 10:45am and by the time they opened for lunch at 11:30, there was a line of 20+ people. Our lunch cost only ¥800.
- After lunch we took the train to Ueno and spent some time in the Tokyo National Museum and Ueno Park. We also visited Shinobazu No Ike Bentendo Temple which is a cool temple right inside Ueno Park. While at Ueno Station, we picked up our JR passes & seat reservation tickets and also put ¥5000 on our Suica. We really enjoyed the fact that you could charge your mobile suica at a convenience store using cash - we didn’t have to worry about finding the machines in train stations with the phone holders.
- After that we walked through Ameyoko and did some shopping - I bought a super cute used Coach purse for around $65. Ameyoko seemed to have a lot more products than when I was there in 2016… but I also had more money this time around so maybe I was just paying more attention!
- We took a train back to our hostel to drop off our shopping bags and change. We took a train to Shinjuku to go to New York Grill for our dinner reservation. New York Grill is on the 52nd floor of the Park Hyatt hotel in Shinjuku and the views were incredible. We got the Spring Harvest 5-course dinner which included wagyu and it was easily the best beef I have ever eaten in my life - so juicy and buttery. The cost was ¥23100 per person (~$175).

Day 4: Shibuya/Akihabara - We woke up and took a train to Shibuya to see Hachiko, go to the Starbucks near Shibuya Crossing, and shop at Shibuya109 and Uniqlo. It was hard for me to find any clothing that would fit me at the stores in Shibuya109 (I am a US 8/10 which is basically considered plus size in Japan) however I was able to find a cute t-shirt at Punyu's in Shibuya109 and two dresses at Uniqlo. I also bought sneakers at the Skechers store.
- I also went to the Mega Don Quijote in Shibuya and spent about $102 on candy and snacks to bring home. Lots of fun KitKat flavors here but it was very crowded and hard to walk around with my extremely full baskets.
- We took a train back to our hostel to drop off our shopping bags and had a quick lunch via conbini.
- We took a train to Akihabara and bought some souvenirs and gachapon, and walked all the way back to Ameyoko where I spent another $90 on candy and snacks to bring home.
- We took a train back to our hostel to drop off our shopping bags and change. We took a train to Ginza to go to Tempura Kondo for dinner (a Michelin starred tempura restaurant). We got the Yomogi dinner which was ¥23100 per person (¥25410 after fees, ~$195). The staff were amazing - they noticed that my sister was left-handed and set up her plates/silverware as such which was really observant. We loved the dinner, however there was WAY too much food for us; the staff recognized we were starting to get full, and asked us if we would be able to eat the next course which was ten-don. I was really concerned about being disrespectful and wasting food, so I was really glad that they noticed and asked!

Day 5: Asakusa/Ginza/Omoide Yokocho - We woke up and took a train to Asakusa to see Sanja Matsuri. We shopped at the small souvenir shops on Nakamise-dori and had street food at the festival for lunch. I bought an awesome goshuincho here with a wooden cover and foxes on it.
- After being completely overstimulated, we took a train to Rikugi-en Garden to escape the crowds. This garden is a little off the beaten path, just outside the big city, but it is completely gorgeous and so worth the trip.
- After the garden, we took a train to Ginza to window shop and go to our reservation at Higashiya Ginza for wagashi & tea pairing. We had 5 wagashi paired with 5 teas that were amazing. It cost ¥4500 per person.
- After this, we walked to Mitsukoshi Ginza to explore the basement food floor and grab dinner. They start to discount the food as the stores near closing time, so going for a late dinner can save you a couple yen! After eating on their rooftop terrace which was beautiful, we walked to Patisserie Sadaharu Aoki Paris Marunouichi to grab dessert then headed back to our hostel to drop off our shopping bags.
- After we ate, we took a train to Shinjuku to walk down Omoide Yokocho. We had a drink on the third floor of Bar Albatross - highly recommend! The alleyway is a lot shorter than I imagined, and very crowded as you would expect. I probably wouldn’t go back here in the future but I am glad we visited!

Day 6: Harajuku - We woke up and took a train to Harajuku to visit Meiji Shrine and got our first goshuin.
- After visiting the shrine, we walked back to Takeshita-dori to get lunch at Afuri Ramen. They’re known for their ramen with yuzu in their broth and this was one of our favorite meals during our trip. We got there about 20 minutes before they opened and were ~8th in line. By the time they opened, the line was stretching around the building!
- We walked down Takeshita-dori and shopped, then went to our reservation at Mipig Cafe (mini pig cafe). This place was so cute! You can make reservations for 30 minutes or 1 hour where you sit on the floor and miniature pigs will come and sit on your lap. You weren’t allowed to pick the pigs up and the staff were super kind and handled the pigs well. A 30 minute reservation was ¥1800 per person.
- After the pigs, we walked down Omotesando and took the train to Shinjuku to get dinner on the food floor at Isetan.
- After we ate on the rooftop terrace (beautiful once again), we headed back to our hostel to drop off our shopping bags. We took the train to Shibuya to go to our reservation at Shibuya Sky. We got the package where you get an admission ticket including a 50 minute reservation for the sofa seats and a choice of a mini bottle of champagne or 2 beers per person, which cost ¥5900 per person. The views were unreal and I recommend reserving the sofa seats so you can get amazing pictures. This was a highlight of our trip.

Day 7: Kawaguchiko - This was our first travel day. We woke up early to take the highway bus from Busta Shinjuku to Kawaguchiko. The bus was ¥4400 per person. We were able to get some super clear pictures of Mt. Fuji while we were on the bus and right when we arrived at the station.
- We bought the daily unlimited bus pass for ¥1500 which ended up not being worth it for us because we spent so little time seeing things on the bus route. First, we went to the Panoramic Ropeway. We got there about 15 minutes before they opened and there was already a really long line. By this time, Mt. Fuji was already completely covered in clouds but the ropeway was really cool and we still got some awesome panoramic pictures. They have a shop at the top where you can get ice cream and sit on some swings and take pictures.
- After the ropeway we took the bus back to Kawaguchiko Station to get on the train to go to Chureito Pagoda. The gnats here were INSANE. We climbed up the ~400 or so steps to the top and got some cool pictures as well as our goshuin.
- We walked back to the station and took the train to Fuji-san Station and walked ~30 minutes to Kitaguchi Hongu Fuji Sengen Shrine. The wooded walk up to the shrine is absolutely gorgeous. We got goshuin here as well.
- After the shrine, we walked and took a train back to Kawaguchiko Station to eat a conbini dinner, do last minute shopping, and catch our bus back to Tokyo.

Day 8: Kanazawa - We woke up early to catch our 6:16am shinkansen to Kanazawa. Once we arrived, we bought the daily unlimited bus pass and took the bus to Omicho Market where we bought some souvenirs and food.
- We walked from Omicho Market to visit Ozaki Shrine and Oyama Shrine and got goshuin at both places. My sister realized she lost her wallet at this point, but a kind soul had turned it in to the staff at Ozaki Shrine!
- We took the bus from Oyama Shrine to Kenroku-en Garden, where we walked around, took lots of pictures, and sat down for lunch at a local restaurant.
- After lunch, we took the bus to the Higashi Chaya district where we shopped and took pictures.
- After that, we took the bus back to Kanazawa Station to catch our 5:57pm shinkansen back to Tokyo.

Day 9: Kyoto Day 1 - We woke up early to pack our overnight bags and catch our 6:21am shinkansen to Kyoto, where we would be staying for one night. We decided to keep our reservation at our Tokyo hostel because 1) it was super cheap and 2) we would be able to leave all our suitcases and belongings in our rooms for when we returned to Tokyo.
- When we arrived in Kyoto, we headed straight to our hostel to have them hold our bags before we could check in later that day.
- After that, we took a local bus to the Arashiyama area. It was absolutely packed with school groups and tour buses. We walked up Saga-Toriimoto Preserved Street to Adashino Nenbutsu-ji Temple where it was much quieter. We got goshuin and visited the small and private bamboo grove behind the temple. We walked back down Saga-Toriimoto Preserved Street towards Togetsukyo Bridge where we stopped and sat down for lunch then continued shopping along the way. I got lots of uji-matcha here.
- We walked back to Arashiyama Station and took a train to Kiyomizu-dera where we got goshuin, then walked to a small rooftop bar overlooking the Hokan-ji temple and pagoda and got awesome pictures and had dinner.
- After eating, we walked back to our hostel and checked in, then slept for the night. Our 1 night stay cost ¥5000 per person.

Day 10: Kyoto/Nara Day 2 - We woke up super early to check out of our hostel and head to Fushimi Inari. Our hostel held our bags while we went sightseeing for the day. We arrived around 6:45am which was so worth it - it wasn’t crowded at all and we got lots of pictures of the shrine and torii gates without any people in them. We walked for about 30 minutes up the mountain then headed back to Inari Station to head to Nara.
- Once we arrived in Nara, we spent some time with the deer then visited Todai-ji and got goshuin.
- After that, we took a train back to Fushimi Inari to visit the souvenir shops which had not opened yet when we had gotten there earlier that day. We shopped and got goshuin, then headed to Sannenzaka/Ninenzaka to shop. We visited Ryozen Kannon to get pictures (they had closed by the time we got there) and walked down Ishibei-koji Lane and Hanamikoji Street.
- After that, we walked back to our hostel to pick up our bags. We took a taxi to Kyoto Station to catch our 7:36pm shinkansen back to Tokyo.

Day 11: Tattoo - My mom and I went to a tattoo shop near Shibuya to get matching tattoos on our wrists, then my mom spent the rest of the day shopping in Shibuya with my sister while I got another large tattoo on my shoulder.

Day 12: Yokohama/Kamakura - We woke up and took a train to Yokohama where we walked the Minato Mirai 21 area and shopped and had lunch at World Porters.
- After lunch, we took a train to Kamakura. First, we visited Hase-dera and got goshuin. I got lots of pictures of the hydrangeas even though they weren’t in full bloom yet.
- After that we walked up the road to Daibutsu and got goshuin. We walked back to Hase Station and took a train to Kamakura Station and walked the pedestrian pathway in the center of the road to Tsurugaoka Hachiman-gu. We got our goshuin and were able to observe a small part of a traditional Japanese wedding that was happening at the shrine, which was really cool.
- After that, we walked back to Kamakura Station and took a train back to Tokyo.
- We spent the night packing. I ended up completely filling my suitcases with my souvenirs and had to put even more in my carry-on bag.

Day 13: Travel - We woke up and headed back to Senso-ji to grab our goshuin, since it was too busy when Sanja Matsuri was happening the week before.
- We took a train back to our hostel and checked out around 11am, then made our way to Tokyo Station to take the Narita Express back to the airport.
- We arrived at the airport with plenty of time before our flight so we could get last-minute souvenirs. I found a couple flavors of KitKat and Hi-Chew here that I could not find in Donki/Ameyoko - but I could not find Beni-Imo KitKat which is the flavor that I was looking for! :(
- We flew back to O’Hare and drove home from there.

I'm on edge with the new supply system.
On one hand, I do like how fleshed out it has become. Supply coming from hubs and ports, railroads and sea access connecting those nodes to the capital and what not.
On the other, I hate how supply from a hub or port is spread out into tiles/provinces where it's not needed. For example, level 1 railway or river access gives you 15 supply, but that supply is spread out 4 tiles wide (if possible). Without trucks or modifiers this supply is concentrated in the center. Motorizations brings this supply further out.
My problem is that hubs and ports shouldn't be sending supply to tiles/provinces that do not need it. Meaning there is a lot of wasted supply just spread out for no reason. Imagine your troops going hungry because the logistics department is sending food and water to a forest and dropping it off to rot and spoil.
I don't want to hate this new system. With hubs being 20000 IC, I played the game like the developers wanted me to. Build up railway and increase motorization. That didn't do much though. Even with building up railway to increase the supply cap from the hub the supply still spread out in provinces with no troops and therefore no use. Trucks only try to level the supply out further from the center - not increase it. Even with +100% infrastructure it didn't do much.
What I don't understand is the supply in your capital. I could build 400 factories and have the capital supply say 170 supply cap but that doesn't really show in the tiles.
What I've come to understand is that the reason Germany looks so green and Russia so blue is because the number of supply hubs that overlap each other's range. You can go into Germany and find several sources of supply. So no wonder one province gets 10 supply because there's 4-5 hubs bringing supply there. In Russia, you may be lucky to get two.
But if this is the case and the only real beneficial way to increase supply is with building more hubs or ports, why did Paradox make it a point to make hubs so expensive? I've even heard some of the devs wanted there to be no supply hubs to be buildable.
I've seen some argue that ports are a problem because you can build so many of them for less than a hub and get a lot of supply. But why punish the player for fixing their supply issue? Isn't that the point of this new game mechanic? To flesh out the supply to troops and make it more interesting and fun?
Armies will grow bigger. More supply will be needed. More hubs and railroads will need to be built. I don't understand why a front needs to stall for 3-4 months for a supply hub.
TL;DR
- I think supply in provinces without troops to consume it is wasteful making improving railways and motorization kind of fruitless effort. Meaning supply hubs and ports are the only real game changers. Perhaps supply hubs with their range provide supply as a State supply stat so no supply is wasted. Kind of like population size and air transports provide state-based supply but aren't distributed per tile but go straight to the troops that need it. Depending on where the tiles are is where each State gets supply from a hub.
- I do like how supply is very much relevant more so than previous versions of the game, but right now it seems like Paradox wants it to be a forever issue in the sense that you can't really fix your supply situation without hubs and ports and we'll make hubs 20000 IC so it's much harder.

| Hello and welcome back to SAR! First off, I apologize for the hiatus in blog posts but had some things pop up and now I am clear! Alright off to the good stuff: For most things in life there are few reasons why one person of a particular sex or race would be more well suited to a particular job than another person of a different sex or race. Afterall, the quality of a cup of coffee would be just the same from a Black female barista with 10 years of experience as an Asian male barista with 10 years of experience. But medicine is a bit different; we do see differences because of sex or race that are important to understand, acknowledge, and factor into the diagnosis. Obviously a male isn’t going to have pregnancy on the differential diagnosis and prostate cancer isn’t a factor for female patients. The differences in Pharmacokinetics and Pharmacodynamics of a drug, a disease, or treatment can vary wildly between the sexes or between races. So today I want to look at the differences that we are only now starting to realize and the emerging role of Pharmacogenomics as the next horizon for medicine. The processes that move drugs from a pill into your body are broadly referred to as ADME but there is a hidden L step that is just as important to talk about as well. The lifecycle of a drug, LADME, are the five major processes that affect a drug inside the body. Let’s look: - Liberation - When you swallow a tablet, the pill as a whole isn’t magically absorbed through the intestinal wall, it has to break down and liberate the drug from the dosage form. Liberation is the process of how the vehicle (what the drug is administered as) is broken down so the drug is free floating inside the body. This is also where we can see differences in timing such as extended release or immediate release products. It also governs other forms too like tablets you can chew or patches you apply onto the skin.
- Absorption - Now that the drug has left the tablet, it needs to move from the GI tract space into the body proper. The mucous membranes inside the intestine have a HUGE surface area allowing for many substances to be absorbed quickly and efficiently. How well a drug is absorbed plays a major factor in how well the drug’s action is. What if you're administering an oral (PO) antibiotic but the patient has diarrhea and they move the drug too quickly through the intestines? Can we be sure it was all absorbed? This is why we have alternative administration pathways like intramuscularly (IM), subcutaneous (SubQ), intravenous (IV), rectally (PR), vaginally (PV), optically (OU), and more. Each gets the drug inside the bloodstream where it can then get to the place it needs to go.
- Distribution - Speaking of getting where it needs to go, we have distribution. Once the drug is absorbed and dumped into the bloodstream, it needs to go to the place where the receptor, channel, or structure is for it to have action. Some places are easier to get to like highly vascularized (lots of blood vessels) areas like skin, the liver, and kidney while others are a bit harder such as the heart tissue, lungs, and brain.
- “How does the drug know where to go?” It doesn’t! It will go everywhere in the body but that doesn’t mean that it will have action there. One good example is with Carbidopa, a dopamine receptor agonist used for Parkinson’s (oh look a post!). When a person take’s Carbidopa orally, it's absorbed into the blood and starts to distribute throughout the body generally. We want it to go to the brain to alleviate Parkinson's symptoms but it can also activate dopamine receptors in the periphery where it causes dizziness and constipation (I highly recommend reading the post if you want to learn how).
- Metabolism - Eventually the drug has to leave the body and metabolism is the first step in that process. Metabolism has two phases: phase 1 is responsible for deactivating the drug by modifying the structure of the drug in a way where it cannot fit inside the receptor anymore. Phase 2 takes the deactivated drug and makes it very water soluble so that it can leave the body extremely easily. The rate of metabolism is dependent on many different factors and is highly specific to each person which is why we do large pharmacokinetic studies in drugs before they are ready to be on the market. Failure to understand how a drug is metabolized can lead to toxicities from drug accumulation OR give too little a dose because the drug is metabolized so quickly.

Elimination - The last step in the drug lifecycle is elimination or how it physically gets out of the body. The majority of drugs leave via urine of which the kidney is responsible for that process. Having a working kidney is key for correctly dosing a drug because an underworking kidney may not eliminate the metabolized drug fast enough causing it to accumulate. There are other pathways too; fecal excretion (through biliary elimination) is another major route but some drugs can be eliminated via the lungs (such as alcohol, which is why it can be detected in a breathalyzer), through breast milk (which is why understanding drugs in pregnancy is SUPER important), sweat, saliva, and sebum. Obviously there are major differences in LADME between males and females—after all it's hard to insert a drug vaginally in a male. But now that we have a general idea of what the drug is going to be doing, let’s dive into each section and talk about the sexual and racial differences we see at each step. Liberation—”Liberty, equality, fraternity, or death; - the last, much the easiest to bestow, O Guillotine!” When we think of liberation I want you to think of two things: getting the drug out of the formulation (such as the tablet or capsule breaking down in the stomach) and where that breakdown happens. These two factors are hugely important when we think about how the drug will eventually get absorbed—after all if you can’t liberate the drug correctly then there is no chance that it will be absorbed and then be utilized by the body. Here we are going to look at the most basic of them: Immediate Release (IR), Delayed Release (SR), and Controlled Release (CR) also known as Extended Release (ER). The main differences between these different forms is how quickly the tablet moves from a solid lump sitting in your stomach to dissolved particles. The slower it releases then the slower the drug dissolves into your stomach and the slower it will eventually get absorbed. Initially all drugs were formulated as Immediate Release because we had little understanding of the utility of longer dosing formulations as well as we didn’t have the technology to do it. There are a whole bunch of different types of Extended Release types each with different uses and drawbacks and choosing which is best for your drug is extremely important for deciding what works best. At the end of this section is a diagram that shows all the different kinds of formulations.

The graph is divided into three zones with two very important barrier points: the Minimum Effective Level (MEL, aka MEC) and the Minimum Toxic Level (MTL aka MTC). Essentially if the curve of the line, representing the concentration of the drug in the body, is below the Minimum Effective Level then there isn’t enough drug to exert its action and we have Subtherapeutic levels in the body. Over time, the tablet dissolves more and more and the concentration continues to rise more and more and we pass the MEL into the Therapeutic Window where we see the action of the drug. In general we can say the higher you are in the Therapeutic Window, the greater the therapeutic effect there is or in other terms, the higher the dose the bigger the effect. Finally if you go above the Therapeutic Window you pass through the Minimum Toxic Level and enter into drug toxicity levels. Now, toxicity is a bit of a harsh word because this doesn’t mean we are going to see death but rather side effects due to too great of an effect of the drug. For example, say you are taking a drug that lowers blood pressure but due to changes in the tablet liberation the concentration rises above the MTL and your blood pressure drops a little too much and you get dizzy.

Let’s say that we have 100mg of a drug that we are trying to put in the body. Using an Immediate Release formulation would result in a much quicker liberation and thus absorption into the body but we can see that it peaks very quickly and then drops off very quickly. So you may only get a few hours of usefulness out of the drug. Compare this to the Controlled Release system which peaks much slower but also declines much slower too, remaining much more Therapeutic for a longer period of time. We can see this in Adderall used for ADHD—most people will take an Extended Release (ER) formulation in the morning so that they get many hours of sustained therapeutic benefit while they are at school or at work. You can see one method of how the beads inside the Adderall capsule are made—called Pelletization. Essentially an inert core is layered with different chemicals/drugs to build an “Onion” of layers [insert Shrek reference].
- So wouldn’t extended release always be better because we avoid the toxic levels of a drug? Not necessarily. To give an example of one of my patients, this person was a 911 operator who would take their Adderall XR every morning at 8:30am and work from 9am to 6pm every day. For them, their Adderall would start to wear off at 4pm, so they would get 7 hours of benefit. But what abouts the last two hours of the work day? They can’t be distracted in their job else it costs lives and taking another Adderall XR would mean they are wired until 11pm. No good. This is where the utility of IR formulations come in—this person can take an IR dose at 4pm when the Extended Release starts to wear off and boost their levels back into the therapeutic range and get 3 or 4 hours of benefit but have it be gone by the time they go to bed.

Well some drugs are more effective if they are absorbed later in the digestive tract where it is less acidic versus the stomach. So some drugs may have better absorption if we can release them further towards the intestines than immediately dumping them into the stomach for absorption. This is where the utility of Delayed Release (DR) systems comes in. Drugs that are considered Acid Labile or sensitive to acid else they degrade are preserved better if they can liberate from the tablet in a less acidic environment. A great example of this is Omeprazole (Prilosec) which is used for decreasing the production of stomach acid to treat acid reflux or GERD—problem is that Omeprazole itself is very Acid Labile and if it enters the stomach unprotected then it degrades fairly quickly. This leads to the use of Omeprazole DR formulations where it is coated in substances that resist acid breakdown like Shellac or Sodium Alginate, thus allowing for the Omeprazole to be liberated in the small intestine where it is more likely to be absorbed.

Well one of the major differences between males and females is that females have a more acidic stomach than males making their stomach environment much more acidic. In addition the Gastric Emptying Time, or how long it takes for substances to move from the stomach into the small intestine for absorption, is significantly longer in females. These two factors mean that females taking a drug that is Acid Labile are more susceptible to degradation in a female than in a male. This means the utility of an Enteric Coated drug which would resist stomach acid would be much more beneficial in a female than in a male. Let’s look at a specific example: the drug Carbidopa is used in Parkison’s to treat the tremors associated with the disease. In younger females, the peak blood concentration for Carbidopa is 22 minutes later than males of the same age. As a female ages and goes through menopause that time to peak becomes more and more similar to males meaning not only are we seeing variability in sex but also in age.
- Another example is looking at the effect of a drug's natural acidity with the relative acidity of that person. Ketorolac (Ketorol) is a non-opioid pain medication used for moderate-severe pain in people where an opiate may not be preferred. The problem with Ketorolac is that it is extremely acidic naturally and it can cause ulceration of the stomach lining if someone has a very acidic stomach environment like a diet filled with acidic food. What we find is that males can have a Ketorolac dose 36% times higher than females because their stomachs are naturally less acidic meaning that they can handle a more acidic drug without the negative side effects like acid reflux or ulceration.
- On a racial standpoint we see similar outcomes. It is reported that individuals of Asian descent have a lower acid output compared to Occidental patients. Part of this can be related to the relative smaller body habitus (weight and height) in the Asian population but there is also a decreased expression of acid-producing tissues in the Asian population than other races. When quantified it appears that Asians have about 60% the acid secretion of Caucasians which is a clinically significant difference when considering drugs that need a more basic environment. For example, Levothyroxine (Synthroid) is taken one hour before breakfast because we want the least acidic environment possible, which happens when the body has no food in it. What we find is that Asian populations need up to 55% smaller doses of Levothyroxine compared to non-Asians. Kinda interesting eh?
Is that a polymorphism in your pocket? Originally I was going to look at each part of LADME individually but kept running into a problem when trying to separate the drug processes into silos. While Liberation is distinct from the rest of the process, ADME cannot really be separated into its components in a way where I can tell the story efficiently. As such we will focus on the poster drugs that are significantly affected by race and sex and then talk about the larger implications in therapeutic development. Now clearly there are drugs that are used in one sex over another, for instance you aren’t going to find Atosiban used to delay labor in a male or Sildenafil (Viagra) for erectile dysfunction in a female. Now that being said they do have other uses but you get the point—so if I talk about a drug please know that I may be talking about a specific indication as well. Unlike liver failure or an infection, you won’t really see any symptoms of high blood pressure but after a decade of not treating it people do develop critical end organ damage: heart failure, kidney damage, liver damage, etc. Imagine that your veins are like a garden hose; if you put your thumb over the end of the hose the velocity of the water coming out of the hose increases meaning that there is more force. So if you have higher blood pressure, you have higher force hitting your organs. So the medical community has developed countless ways of lowering blood pressure to help save our organs and we can group those medications into a couple categories. - For the majority of the population we follow a pretty tried and tested protocol of drugs: first up we would use Diuretics which physically decrease the amount of fluid that is in the blood vessel. In the hose analogy this is like if you had a leak halfway up the hose lowering the amount of water exiting the hose at the end. Now there are dozens of diuretics but most people are familiar with Furosemide (Lasix) and how it makes people urinate more (y’know, to get rid of the fluid). If someone needs another agent we turn to drugs like the Angiotensin Converting Enzyme inhibitors (ACEi) or Angiotensin Receptor Blockers (ARBs). The ACEs (like Lisinopril, Enalapril, or Benazepril) and ARBs (Valsartan, Olmesartan, etc.) work by inhibiting the ability for a protein, Angiotensin, from connecting with the blood vessel wall. Angiotensin’s main job is to make the blood vessel smaller which would increase blood pressure—these drugs inhibit the action of Angiotensin thus preventing it from raising blood pressure. And our final category are drugs acting on the heart: Calcium Channel Blockers or Beta Blockers. While generally more helpful in situations like heart failure than hypertension, these drugs slow down the heart’s ability to pump blood which then lowers the force that the blood is acting on the blood vessels and organs. In a way it's like turning the faucet on the garden hose letting out less water into the hose.
- So to summarize, the majority of people will benefit most from a Diuretic (which lowers the amount of fluid in the blood vessel), an ACEi or ARB (which prevents the blood vessels from getting smaller), and/or a CCB or BB (which lower the output of the heart).

These drugs are normally used in situations where the blood vessel is obstructed, such as during a heart attack or stroke, due to a clot. Administering a Venous Dilator would expand the blood vessel allowing the blood to wiggle around the clot and relieve the symptoms of the heart attack or stroke. Two of these drugs are Hydralazine and Isosorbide Dinitrate which are used by all sexes and races to prevent the progression of a heart attack and stroke which have very good success. Both of these drugs are fairly old: Isosorbide Dinitrate was discovered in 1949 while Hydralazine was discovered in 1949 and showed fairly good efficacy for decades.
- Enter two doctors, Jay Cohn and Peter Carson, who wanted to market a new combination drug Hydralazine/Isosorbide Dinitrate (BiDil) in 1989 for congestive heart failure. The FDA said sure, go do a trial and so the duo set out to prove that BiDil was a good option for the general population. They tested their combination in a trial called Vasodilator-Heart Failure Trial (V-HeFT) against the gold standard Enalapril (ACEi) and found that BiDil was…bad. Like really bad, in fact they had to stop the trial early because it would have been unethical to keep people on the BiDil rather than on the known treatment. So V-HeFT didn’t show that BiDil was a good option for the general population except among one subpopulation: Black patients. When you look at the use of Enalapril (and other ACEi) in Black patients we find that the efficacy is quite low. As it turns out, people of African descent have a lower response to drugs like ACEi and ARBs compared to non-Africans meaning that they are worse options for a Black person. So a patient that wasn’t responding well to a Diuretic and needed a second agent would get little help from an ACE/ARB drug. And at this point in time there were no CCB or BB!

On the recommendation of one of the FDA’s advisory committees they stratified the data on racial lines and discovered a significant difference in response among Africans vs non-Africans. They started a new trial and enrolled 1050 African men and women showing a 43% decrease in mortality and a 39% reduction in hospitalization against placebo. They reapplied for BiDil and in 2005 the FDA approved the first ever race-based drug. To this day if you go to BiDil’s prescribing page in a drug database it will include a specific race designation because it’s only approved for a specific race. Importantly too national organizations like the American College of Cardiology and American Heart Association endorse this race based recommendation for Black patients.
- Now this is great that we have a recommendation for a group of people that is superior to the normal regiment but there are a few complicating factors to consider. Firstly the term ‘Black patient’ isn’t really scientific—how Black does a patient need to be in order to qualify for BiDil? What kind of Black as well: African, Latin American, Caribbean, etc? Likewise this means it has to be self-reportable but what about a light-skinned Black person that was adopted and didn’t know they had Black genes? It's all very…unscientific. Recently there has been push back against this explicit race-based recommendation since other effective drugs do exist (like the CCB and BB which were discovered in 2006, one year after BiDil). There was a new study in 2022 from UCSF that talked about the role of prescribing based on race but personally I think there are too many confounders (influencing factors) like socioeconomic status to accurately say BiDil should be done away with. BiDil was shown to be effective in the Black population but how it gets applied is another story.

This process of deactivating a drug happens during Metabolism which primarily happens in the liver via the Cytochrome P450 Enzymes. These CYP enzymes are like little factories that take up a drug and make slight modifications that remove its ability to affect the body. Each CYP enzyme is best suited for certain drugs while some enzymes, like CYP3A4 and CYP2D6, are able to handle a higher percentage of drugs. You can see in the pie chart above that some enzymes are more important in metabolism than others. - That being said, the distribution of the CYP enzymes is not constant from one person to another. For example, in females they have a higher distribution of CYP3A4 than males which can be thought of them having more deactivation factories than males. This means if we were to give the same dose to a female and a male the female would metabolize the drug much faster and thus have a decreased effect. This is incredibly important because CYP3A4 accounts for 30% of all drugs including birth control, many psychiatric drugs, and chemotherapeutics meaning that females would need higher doses than males, in theory. The opposite is true for CYP2E1 which is higher in males than females and is a metabolizer of ethanol (minor) and caffeine (major) which means that higher doses of both would be needed to have the same effect male vs female. Neat eh? Like there are sex differences for CYP enzymes we are starting to discover racial differences in CYP enzymes: we have identified Polymorphisms or small genetic variability among the genes that encode the CYP enzymes. Truthfully this isn’t surprising, we know there is genetic variability in genes that affect hair or eye color, so it's not too surprising that the CYP enzymes are equally affected. What is interesting is that the racial differences in CYP enzymes can lead to wildly different enzyme activity levels that lead to clinically significant differences in drug response. Or in simpler terms, some races respond better to some drugs because they have different liver enzymes!

Quickly let's take a look at the metabolism: we can see that Amitriptyline is metabolized by two enzymes, CYP2D6 and CYP2C19. If it is metabolized by 2D6 first then it converts into an inactive metabolite that is eventually removed from the body. However if it first goes through 2C19 then it is converted to an active metabolite, Nortriptyline which would extend the action of the drug than if it went through 2D6. But if it does go through 2C19 then it will be handled by 2D6 into an inactive metabolite (bottom right), so either way we need 2D6 to get rid of the drug.

Well if someone is a 2D6 poor metabolizer then they would be unable to clear the drug quickly and so it would hang around in the body longer—so more anti-depression activity (and side effects). We find that up to 10% of Caucasians are poor 2D6 metabolizers meaning that they would have a better response to Amitriptyline than those who are normal metabolizers. Likewise we find that up to 51% of Asians are intermediate metabolizers which has led to an overall increased response of 74%! The flip side to this are the Ultrarapid metabolizers who would clear the drug extremely quickly through 2D6 thus leading to a decreased response to Amitriptyline. Up to 29% of Subsaharan Africans are found to be Ultrarapid metabolizers meaning that Amitriptyline (and other drugs going through 2D6) is the wrong option for them.

Remember that 2C19 converts the drug into an active metabolite which would extend the activity of the drug—if you don’t go through 2C19 then you’d have a decreased response. So patients who are Oceanian have a whopping 89% chance to be an intermediate or worse metabolizer and so a much MUCH higher chance of decreased response to Amitriptyline. Compare this to Caucasian patients who have a 32% chance to have increased activity on 2C19 meaning a potential increased response to Amitriptyline.

No, I am glossing over some of the finer details but I wanted to show how racial differences are a major factor in how drugs affect the body. Imagine if you had a population that had an 89% chance to have a decreased effect on birth control? Or what if you were a carrier of the HLA-B*5701 gene which means you are horribly allergic to the anti-HIV medication Abacavir (like up to 20% of Indians)? This is where the next generation of medicine comes from and the Clinical Pharmacogenetics Implementation Consortium is aiming to have their CPIC genomic guidelines become standard practice. In fact I am seeing more and more patients getting their liver enzymes done so we can see what kind of response they would have, especially if they have failed multiple drugs for a condition. Take a look at the guidelines page and see how many drugs are incorporated into the recommendations.
Regardless, pharmacogenetics has major impacts on how we treat patients every day but also may be a factor in some of the clinical disasters that we see. Afterall, 53% of Africans have decreased metabolism through CYP3A4 which is the major metabolizer of Fentanyl which may be a contributing factor why Blacks face higher overdose rates. If you haven’t, consider getting a pharmacogenomic test (many insurances will pay for it now) and unlock some secrets of your own body! I know Wilton isn’t technically Elk Grove, but it’s close enough, right?
The only restaurant I’ve heard of out there is the Sheldon Inn, which looks absolutely lovely, but we’ll have a toddler with us. Granted she does really well in restaurants and we make sure that she is quiet and contained, but I don’t want to bring her to a more upscale place out of fear that we would be seen as an inconvenience or acting inappropriately.
My in-laws will be in town, and my FIL is very into the “small town rustic” thing, so we thought a quick trip for lunch in Wilton would be just his thing!
I’ve driven through a few times, but I guess I’m not looking in the right places. I’d like to avoid Mexican food because the in-laws are from the LA/San Diego area and are used to really good Mexican food/have it all the time.
Any advice is very much appreciated! Also, if anyone can recommend an activity to do out there, that would also be so helpful, although we’re not set on doing anything but eat out there.

2023.05.30 01:37 Latticese Three years ago I had a dream about a presentation I watched onboard a UFO and it still bothers me to this day
I found myself in a blank white room without doors. It was cold and I was sitting alone on a single-seat, white sofa while a big screen played out a presentation
Two things that stood out as strange to me was how I was aware of it being a dream and yet I wasn't able to control it like how a lucid state allows.
Additionally, I felt the temperature of the room perfectly. The texture of the sofa was similar to leather and I sank right into it like it's stuffed with gel.
I didn't see any faces or learn any names. The voice sounded hard to distinguish as male or female and was somewhat robotic. The presentation involved imagery that was strictly familiar to me so I didn't get to see any aliens in it either
What makes think the room was a ufo was it's odd oval shape and the absence of sharp corners. The floor neatly blended into the wall which blended into the ceiling etc
I will try my best to remember what I learned:
Their religion revolves around the pursuit of happiness and the quest for self-actualization and belonging. They believe that true happiness is achieved when who the person thinks they are, who they want to be, and who they're perceived as are one and the same person
According to them, this need is universal across different species, suggesting a pattern in nature
For instance, atoms combine to form biological matter, which gives rise to cells, and cells create living beings with consciousness. Consciousness represents the peak of complexity, but they told me their research has revealed that it continues to evolve into more advanced forms
Collective consciousness forms another chain in the line. A complex form of life called thoughtforms or Egregores in English. They rely on the energy, emotions, beliefs, and thoughts of conscious entities in a wide variety. There is those who feed on negative emotions, positive emotions, beliefs and even abstract things like dreams etc. While we coexist with these beings without full awareness( similar to how nerve cells are unaware of the mind they are a part of) they have realized their profound influence on their society
By becoming aware of these beings and actively promoting positive collective consciousness, they have freed themselves from the harm caused by negative feeding entities. They urge our society to become aware of thoughtforms and restructure our way of life to foster equality and a sense of belonging for all
In their pursuit of equality and belonging, they have implemented a strange system of temporary identities. People can choose specific roles within society, such as a fish farmer caring for an elderly father. They join host families that treat them no differently than any other adopter. To preserve anonymity, and ensure unbiased treatment individuals wear masks and full-body suits for their role. The outfit is the same for every participant who adopts the role. They can switch roles indefinitely, exploring different lives or settle into one they created for themselves. After death, they pass on the identity they created for others. This system has proven instrumental in achieving self-actualization.
Furthermore, their architecture fosters community and social interaction. Park tables, for example, feature small lights that individuals can change the color of to signal their openness to conversation. Apartments have built-in private networks that allow residents to chat with neighbors on the same floor or in the same building etc
In short, their religion emphasizes the pursuit of happiness, self-actualization, and a sense of belonging to improve their collective thoughtform
This was very vital for them to achieve. They say that just how our body's cells don't understand why they get cut or recieve less nutrition sometimes, we have good and bad experiences at seemingly random patterns because of our egregores. By concentrating on feeding the positive energy beings they managed to virtually purge suffering from their daily lives.
This is absolutely bizarre to me but I had to share it to get it off my chest. Another interesting part was about how their economy is designed to achieve equality, less-waste and encourage innovation

The pyramid represents the structure of wealth distribution, ensuring that no class disproportionately grows over others. To explain this concept, let's consider a lemonade stand as an example.
In traditional capitalism, the owner of the stand and resources would receive the largest share of the profit, while the worker is paid a fixed wage, regardless of their contribution to the business. This system encourages the owner to reduce costs, including worker wages, and increase output, often resulting in poor quality products. Moreover, any unsold or unused items go to waste, which harms both individuals and the environment.
In their system, the worker receives a fixed ratio or percentage of the earnings rather than a fixed salary. Owners are legally required to register employees on the fair-pay program, and all transactions must go through the bank. This approach eliminates human involvement to prevent corruption. To maintain transparency and prevent hacking, the AI's decision-making process and transactions are publicly visible through a live-feed. Any hacking attempts or discrepancies between reported payments and actual receipts would be immediately evident. Additionally, the system has robust cybersecurity measures, including compartmentalization of information and regular checks by a randomly selected team of developers
Their government does not control the bank or the payment ratio. It runs automatically and isn't staffed by anyone but the publicly transparent AI. Instead, people get to vote on the appropriate ratio for different skillsets. The government's role is to safeguard the system and provide public surveillance of the bank's activities. Innovation and education are highly valued, with scientists, educators, and engineers receiving the highest salaries and belonging to the top layer of the pyramid. They are paid in an exclusive currency that is priced at three times the standard currency
While the private sector has full control over means of production, raw resources have shared control between the private and government sectors. For instance, if a company discovers a copper mine and develops an effective extraction/purification method, they receive a fixed percentage of the profit, while the government sets a fair price for the resources based on extraction costs. People can buy these resources and use private sector machinery and blueprints to create items of their choice.
There are machinery and blueprints available to the general public for free, but they are limited to basic designs necessary for survival. This restriction prevents exploitation during emergencies. Businesses, therefore, focus on innovation and improving designs, ideas, patents, and machinery to produce items more effectively.
Their system has a strict made-to-order process for purchasing items, reducing environmental damage from unsold goods. Only perishable items like food, vital medicines and digital-goods are allowed to have surplus quantities. If a food item nears its expiration date, it would be offered at an extremely low price rather than being disposed off. This discourages businesses from over farming
Additionally supermarkets don't use packaging to sell food. To minimize harm to the environment, they bring in their own reusable containers to fill up from their product of choice, which is stored in large containers within the shop. Weight/volume determines the cost
In politics they don't have a party system and prefer direct democracy over representative democracy because they see the existence of a middle-man as a doorway for corruption. Their people get to vote on individual issues just like what happens in Switzerland. One difference is that they're required to do a lot more than simply tick yes or no. They believe that education is a vital part of successful democracy, therefore the voter has to write a short essay weighing the pros and cons behind their decision to demonstrate that they've learned enough before making a decision
The consequence of this system is that it takes very long to collect and process votes (It's done by AI which also has it's process live-streamed) The person is required to write it while at the voting center (to prevent copy-pastes) The booth provides them with a PC which has all the gathered points and research supporting each side of the decision.
To reduce the time it takes to submit and process votes, they prioritise those who are most affected by a decision. For example if it's about women's reproductive health then only women can vote. The rest of the population can still participate by adding to the information stockpile for the voting booth. Another example is when it comes to war, only those who are enlisted are allowed to vote in favour of it to discourage an eagerness for violence
Just like the bank system the government is also unstaffed. There is only an AI controlling humanoid drones to complete tasks. It's also publicly surveyed (I find it interesting how the government is watched by people rather than the other way around) they also have a randomly selected maintenance team to work on the program running it. They're taken to work immediately to prevent anyone from bribing them. Even more public surveillance goes into their work
There is lots more finer details about their society but I unfortunately forgot it after years passed. I might remember more with time but this post already got ridiculously long. After the dream ended I just woke up home and was in a cold sweat because of how bizarrely detailed it was. My dreams are usually random junk that I forget completely the instant I wake up
