I’m sure I’m not the only one that’s extremely impressed by NL’s trivia knowledge.
I understand that at this point he’s practicing and learning by doing trivia on stream, but I was just wondering if anyone had any insight on how he learned all of this stuff in the first place?
Clearly he’s an intelligent guy and was an intelligent kid (he was in the chess club or something, right?) but beyond that I was wondering if he’s ever talked about learning all of this stuff because I’m studying some trivia and would love to have all this random knowledge like he does so I’m trying to learn how he did it/does it.
I know I’ve heard him say that for movie release dates he connects it to what was going on in his life at the time, has he mentioned other memory skills like that?
Perhaps he’s just genuinely interested and has been doing trivia stuff for a long time so when you know 5/6 things it’s pretty easy to fit the 6th thing into your existing frameworks?
Anyway, would love if anyone had any input and if this post doesn’t get any traction I’ll make a version of it where I accuse NL of cheating because this much trivia knowledge is impossible and hopefully that will get people riled up enough to explain how he does it in his defense haha thanks!

So long story short I was followed for 30 mins on the road, dude trying to run me off the road, trying to hit my vehicle, yelling that I killed his daughter. Police showed up and no charges were brought against the man. The police didn’t give me much info but I assume he does have a daughter missing/dead if they didn’t bring him in for a psychiatric evaluation. The police calmly told me he was looking for my vehicle type and not me in particular. There was also a true look of pain when the man spoke about the girl.
Information I have-
I live in Thornton, Colorado and so does the man. But his ID showed a different city (unsure where, somewhere south likely Denver area is all i know). His daughter’s name was Paige (spelling not confirmed). The man appeared to be no older than 45 (possibly younger, appeared he could have been an active/ex drug user based on the looks of his skin). So I assume the daughter would be 25 or younger. 30 oldest as I can’t be too sure of the man’s age. But she could be as young as an infant for all I know. He was specifically looking for a gray/silver Ford Fusion. I briefly heard him tell one of the officers when they arrived after him chasing me something about “body never found.”
Another thing, I have to put “ -birgfeld” to exclude that word from google searches due to a high profile crime case of a woman named Paige in Colorado that is unrelated otherwise that is all that comes up.
Just being accused of it makes me very curious about the situation.

I grew up in a poor family, my uncle lived with us and whenever he gets drunk he would always scream at my mom and accuse her for cheating on my dad with my dad’s friend which is also married. I was so young so I never really find out if it’s true or not. He would bang our door and throw our furnitures. My dad is working abroad that time so he never got to see my uncle acted like that.
To protect us from my uncle whenever he gets drunk, they built a metal gate on our bedroom door that we can lock from the inside if ever we need to. - I only realized how crazy this sounds now that I’m older.
I remember one time, me and my uncle were eating at the table. He was mumbling about how I look good for my age and my looks should stay like that forever. Even though I was young, I understood what he meant by that. I was 10 yrs old and by the way he stares at me and how he say those words. I know that he was threatening to kill me.
I ignored it..
He never tried to kill me but he would always give me this hate look.
Again, I was young and I don’t remember everything happened after that.
After few years.. my uncle doesn’t live with us anymore but he lives with my aunt which is few doors away from us.
I was getting ready for school then I heard my aunt screaming outside asking for help
I didn’t recognize her right away because of how scared and devastated her face when she was screaming for help. Found out that my uncle tried to kill himself. He cut his risk so bad that he severed his right hand! I run and called my mom and our family to tell them what’s going on and to ask for help. I couldn’t look out and I shut the door.. once they left, I went inside to their house and I saw a trail of blood coming from the bathroom. There it was, my uncle’s right hand on the bathroom floor.
Weird… I was not scared, I was not disgusted, for me it looks like it was from a movie.
My cousin and I bury the severed hand and moved on. My uncle survived and was released from the hospital. After that, I didn’t see him for years since they advised him not to go back to the place that will trigger his memory of why he did it.
Continued my life and couple of times our house got robbed while we were sleeping
Now, I’m a grown woman and I can’t sleep by myself. I’m feeling anxious that someone might try to robbed or get in my house or a ghost. but I can sleep good when Im with someone else.
I don’t know what’s the connection of me having trouble of sleeping by myself with what happened in my childhood. So I don’t know if I have a trauma or I’m just not used to sleeping by myself.

When a person's two lobes are split (usually to help with seizures), sometimes the two lobes can develop different personalities. With one human, there was a personality who was Christian, and a personality who was atheist. Doesn't this disprove the soul? Like they couldn't split the soul into two halves.
Heard this argument from an otherwise average edgy atheist who was accusing me of being a "brainlet who worships skydaddy" and so forth... But this argument is really pretty strong. Thoughts?

I'm getting so frustrated with not being able to effectively communicate with my SO, not being able to voice my concerns or feel heard. I know I have some responsibility in this as well but I feel like I've tried enough different ways of communicating and received the same sort of response from him so it's hard to even feel comfortable bringing things up.

Any time we argue he's constantly interrupting me, repeating himself, demanding answers to questions that should be rhetorical and won't let me speak until I answer them, and contradicts himself with his "logic".

The most recent example of this is him accusing me of starting a fight because I "got an attitude" (made a face because I was offended by his response) after I asked him what was wrong and he said "what do you think?" In a sarcastic tone. Yet when I explained what I was asking and said his response made me feel like he thought it was a stupid question, he said I should "focus on what was actually said" even though I was somehow responsible for a whole argument without actually saying anything. Make it make sense.

I'm so sick of not feeling safe even asking a simple question without having to anticipate a shitty response.

2023.06.04 16:44 I-am-the-Peel (Spoilers Extended) A Character Analysis of Varys, the fake and lying eunuch

Apologies for the length and lateness of this post, past few months have been quite chaotic and haven't had much access to the internet, I dunno when I'll be back on this sub again after this so until then, more posts from me will be a Dream for Spring Summer (probably)
Over the years I've read a great many theories about the character of Varys, his motivations, his scheming throughout the series and his endgame, but none of them have ever truly sat well with me for how much his character feels simplified. He is one of ASOIAF's most complex characters who I still believe we haven't entirely cracked yet, and I for one still believe even after writing this post up and reflecting on it, there's still some of his character that I haven't figured out.
However, I want to propose an alternative character analysis to the character of Varys, one that drifts away from the more common take on his character - Varys is not a eunuch or a friend of Tyrion Lannister, and has spent his entire life lying about his manhood to deter anyone from thinking that he could father any children, so that fAegon, his true son, would always be protected while hiding.
Let us begin by breaking down each of the lies that define his character.
First, the lie that he was castrated as a young slave and had his manhood thrown into a fire by 'a certain man', which resulted in the fire turning magically blue and 'a voice' speaking out to Varys.
"One day at Myr, a certain man came to our folly. After the performance, he made an offer for me that my master found too tempting to refuse. I was in terror. I feared the man meant to use me as I had heard men used small boys, but in truth the only part of me he had need of was my manhood. He gave me a potion that made me powerless to move or speak, yet did nothing to dull my senses. With a long hooked blade, he sliced me root and stem, chanting all the while. I watched him burn my manly parts on a brazier. The flames turned blue, and I heard a voice answer his call, though I did not understand the words they spoke. "The mummers had sailed by the time he was done with me. Once I had served his purpose, the man had no further interest in me, so he put me out. When I asked him what I should do now, he answered that he supposed I should die. To spite him, I resolved to live. I begged, I stole, and I sold what parts of my body still remained to me. Soon I was as good a thief as any in Myr, and when I was older I learned that often the contents of a man's letters are more valuable than the contents of his purse. "Yet I still dream of that night, my lord. Not of the sorcerer, nor his blade, nor even the way my manhood shriveled as it burned. I dream of the voice. The voice from the flames. Was it a god, a demon, some conjurer's trick? I could not tell you, and I know all the tricks. All I can say for a certainty is that he called it, and it answered, and since that day I have hated magic and all those who practice it. If Lord Stannis is one such, I mean to see him dead." - ACOK - TYRION X
Now there is much from this excerpt to raise complaints and questions about Varys' story;
But the weakest part of all is that Varys says that he "hates magic and all those who practise it", yet he goes to great lengths to protect and look out for Daenerys, and try to join fAegon's forces with hers, despite knowing that she has three magical dragons by her side.
"How many days until we reach the river?" he asked Illyrio that evening. "At this pace, your queen's dragons will be larger than Aegon's three before I can lay eyes upon them." "Would it were so. A large dragon is more fearsome than a small one." The magister shrugged. "Much as it would please me to welcome Queen Daenerys to Volantis, I must rely on you and Griff for that. I can serve her best in Pentos, smoothing the way for her return. So long as I am with you, though … well, an old fat man must have his comforts, yes? Come, drink a cup of wine." - ADWD - TYRION II
Illyrio is Varys' best friend and closest confidant, who he works with to protect Daenerys for much of her life and is willing to crown her as Queen alongside fAegon even though she has magical fire-breathing dragons that Varys, a victim of magical flames, would despise.
Illyrio, the same man who praised R'hllor, a God that Varys despises;
Her brother was waiting in the cool of the entry hall, seated on the edge of the pool, his hand trailing in the water. He rose when she appeared and looked her over critically. "Stand there," he told her. "Turn around. Yes. Good. You look …" "Regal," Magister Illyrio said, stepping through an archway. He moved with surprising delicacy for such a massive man. Beneath loose garments of flame-colored silk, rolls of fat jiggled as he walked. Gemstones glittered on every finger, and his man had oiled his forked yellow beard until it shone like real gold. "May the Lord of Light shower you with blessings on this most fortunate day, Princess Daenerys," the magister said as he took her hand. He bowed his head, showing a thin glimpse of crooked yellow teeth through the gold of his beard. "She is a vision, Your Grace, a vision," he told her brother. "Drogo will be enraptured." - AGOT - DAENERYS I
Neither Viserys or Daenerys were followers of R'hllor at this point, or surrounded by any followers, meaning Illyrio didn't need to bring up this form of prayer to R'hllor unless it was genuine or near genuine on his own part.
It makes little sense that Varys would entertain the company of those who worship R'hllor or magical forms of fire, if Varys truly is traumatised by what seemingly happened to him as a slave child by fire.
As well as Illyrio, this is apparent in Aerys II, who would regularly burn people in the throne room with wildfire, in the presence of Varys;
Frustrated, Aerys turned to the Wisdoms of the ancient Guild of Alchemists, who knew the secret of producing the volatile jade green substance known as wildfire, said to be a close cousin to dragonflame. The pyromancers became a regular fixture at his court as the king's fascination with fire grew. By 280 AC, Aerys II had taken to burning traitors, murderers, and plotters, rather than hanging or beheading them. The king seemed to take great pleasure in these fiery executions, which were presided over by Wisdom Rossart, the grand master of the Guild of Alchemists...so much so that he granted Rossart the title of Lord and gave him a seat upon the small council. - TWOIAF - THE TARGARYEN KINGS - AERYS II
Not only would Varys be present to witness these burnings, but Varys even encouraged Aerys to burn other individuals suspected of treachery, and fuelled both Aerys' paranoia and bloodlust;
He floated in heat, in memory. "After dancing griffins lost the Battle of the Bells, Aerys exiled him." Why am I telling this absurd ugly child? "He had finally realized that Robert was no mere outlaw lord to be crushed at whim, but the greatest threat House Targaryen had faced since Daemon Blackfyre. The king reminded Lewyn Martell gracelessly that he held Elia and sent him to take command of the ten thousand Dornishmen coming up the kingsroad. Jon Darry and Barristan Selmy rode to Stoney Sept to rally what they could of griffins' men, and Prince Rhaegar returned from the south and persuaded his father to swallow his pride and summon my father. But no raven returned from Casterly Rock, and that made the king even more afraid. He saw traitors everywhere, and Varys was always there to point out any he might have missed. So His Grace commanded his alchemists to place caches of wildfire all over King's Landing. Beneath Baelor's Sept and the hovels of Flea Bottom, under stables and storehouses, at all seven gates, even in the cellars of the Red Keep itself. - ASOS - JAIME V
Varys developed a reputation for egging on Aerys' paranoia and encouraging him to execute apparent traitors by fire, so much so that people like Barristan lay chief blame for Aerys' downfall into madness on Varys for exploiting it;
"It is not a question of wanting. The throne is mine, as Robert's heir. That is law. After me, it must pass to my daughter, unless Selyse should finally give me a son." He ran three fingers lightly down the table, over the layers of smooth hard varnish, dark with age. "I am king. Wants do not enter into it. I have a duty to my daughter. To the realm. Even to Robert. He loved me but little, I know, yet he was my brother. The Lannister woman gave him horns and made a motley fool of him. She may have murdered him as well, as she murdered Jon Arryn and Ned Stark. For such crimes there must be justice. Starting with Cersei and her abominations. But only starting. I mean to scour that court clean. As Robert should have done, after the Trident. Ser Barristan once told me that the rot in King Aerys's reign began with Varys. The eunuch should never have been pardoned. No more than the Kingslayer. At the least, Robert should have stripped the white cloak from Jaime and sent him to the Wall, as Lord Stark urged. He listened to Jon Arryn instead. I was still at Storm's End, under siege and unconsulted." He turned abruptly, to give Davos a hard shrewd look. "The truth, now. Why did you wish to murder Lady Melisandre?" - ASOS - DAVOS IV
It makes very little sense that Varys, a eunuch who seemingly hates worshippers of R'hllor and pyromancers for burning people and sacrificing his manhood to the flames as a child, would be best friends with a man who praises R'hllor or encourage his previous King to burn several people, many of whom were likely innocent of the claims of treachery put against them.
But it makes a lot more sense that Varys actually never had a problem with R'hllor or wildfire, and instead chose to encourage Aerys' paranoia so that he would always both seem useful to Aerys and be on good relations with him, to solidify his position of power on the Small Council, given that he was just a young foreign man in Westeros and wholly dependent on Aerys' support to maintain his standing in King's Landing.
The second lie of Varys' character that I wish to tackle is his apparent concern for the wellbeing of children;
"No," Ned pleaded, his voice cracking. "Varys, gods have mercy, do as you like with me, but leave my daughter out of your schemes. Sansa's no more than a child." "Rhaenys was a child too. Prince Rhaegar's daughter. A precious little thing, younger than your girls. She had a small black kitten she called Balerion, did you know? I always wondered what happened to him. Rhaenys liked to pretend he was the true Balerion, the Black Dread of old, but I imagine the Lannisters taught her the difference between a kitten and a dragon quick enough, the day they broke down her door." Varys gave a long weary sigh, the sigh of a man who carried all the sadness of the world in a sack upon his shoulders. "The High Septon once told me that as we sin, so do we suffer. If that's true, Lord Eddard, tell me … why is it always the innocents who suffer most, when you high lords play your game of thrones? Ponder it, if you would, while you wait upon the queen. And spare a thought for this as well: The next visitor who calls on you could bring you bread and cheese and the milk of the poppy for your pain … or he could bring you Sansa's head. - AGOT - EDDARD XV
Varys shows apparent disgust at the murders of Rhaegar's children, Rhaenys and Aegon, at the end of Robert's Rebellion, and the way in which they were brutally murdered.
However, we later learn in the series that Aegon seemingly survived and was swapped with a tanner's child, bought by Varys and swapped with Aegon, so that this bought child could be sacrificed to the Lannisters, who Varys presumed would want to kill Rhaegar's children;
"A true friend, our Lord Connington. He must be, to remain so fiercely loyal to the grandson of the king who took his lands and titles and sent him into exile. A pity about that. Elsewise Prince Rhaegar's friend might have been on hand when my father sacked King's Landing, to save Prince Rhaegar's precious little son from getting his royal brains dashed out against a wall." The lad flushed. "That was not me. I told you. That was some tanner's son from Pisswater Bend whose mother died birthing him. His father sold him to Lord Varys for a jug of Arbor gold. He had other sons but had never tasted Arbor gold. Varys gave the Pisswater boy to my lady mother and carried me away." "Aye." Tyrion moved his elephants. "And when the pisswater prince was safely dead, the eunuch smuggled you across the narrow sea to his fat friend the cheesemonger, who hid you on a poleboat and found an exile lord willing to call himself your father. It does make for a splendid story, and the singers will make much of your escape once you take the Iron Throne … assuming that our fair Daenerys takes you for her consort." - ADWD - TYRION VI
This entire "splendid story" as Tyrion calls it, is full of inconsistencies and unbelievable acts.
The first red flag of this story is the idea that Varys, a former slave child who abhors slavery, would willingly buy another man's young child. Also pay attention to the mention of 'Arbor gold', which has been well documented on this sub for years for how its always a signal of lies, as individuals like Littlefinger mention it in passing whenever they are telling a lie.
The second red flag is the idea that Varys, someone who seemingly abhors the murder and suffering of innocent children, would willingly sacrifice an innocent young child and allow them to be murdered just to spare another, more valuable child. The idea that Varys would also take Elia's child away from her, either willingly or unwillingly on her part, is also questionable.
The third red flag is why Varys chose only to switch Aegon with another child, and not do the same for Rhaenys. Having two imposter Targaryen children raised to believe they are Targaryens could've helped to support their claims, build them up as a rival power in Westeros and also provide a backup option for Varys incase fAegon didn't survive his childhood.
The final red flag is the idea that Varys was able to foresee how the Rebellion was going to end, accurately guessed that Rhaegar's children would be murdered by the Lannisters, knew where and when to find Elia and her children, be able to remove one of her children away from her without anyone knowing, and knew that one of Tywin's men would kill the children so brutally that next to no one would be able to recognise them - the idea story is built on a series of flimsy conveniences and incredible predictive abilities on Varys' part.
If Varys truly cared so much about the protection and well-being of children, he would not have willingly bought and sacrificed an innocent child to be sacrificed in exchange for Aegon's survival, nor would he have abandoned Rhaenys to her fate if he could have prevented it. This means either Varys truly doesn't give a damn about the wellbeing of children and just lies to the likes of Ned and Tyrion about it, or that the entire story about Aegon being swapped with a tanner's child is a lie like some fans believe, or that both stories are lies muddled together to make it more difficult to discern the truth.
Perhaps the truth of Varys' lies is best exposed in this often cited excerpt from ACOK, a conversation between him and Tyrion about the infamous "Shadow on the Wall" riddle;
Varys smiled. "Here, then. Power resides where men believe it resides. No more and no less.""So power is a mummer's trick?" "A shadow on the wall," Varys murmured, "yet shadows can kill. And ofttimes a very small man can cast a very large shadow." Tyrion smiled. "Lord Varys, I am growing strangely fond of you. I may kill you yet, but I think I'd feel sad about it." - ACOK - TYRION II
The question of who Varys refers to in this conversation has been debated for years on this sub. Some believe that Varys was alluding to fAegon, that if people believe he is the real Aegon then he will have power, even if he doesn't truly have power as a common non-Targaryen child. Others have suggested Varys was alluding to Littlefinger, who was once a young small man who rose very high in King's Landing and cast a large shadow over the realm by starting the WotFk through his schemes.
I however believe that Varys is talking about himself.
Varys was once a very small man when he came to King's Landing at Aerys' calling for him to join the Small Council. He was seen as a young ex-slave, without a lordship or house of his own, weak and powerless. But overtime, through his manipulation of Aerys, encouraging his paranoia and own self-inflicted downfall, Varys cast a very large shadow over the empire of House Targaryen in Westeros, and destroyed it forever. What Varys says here to Tyrion is a very veiled threat that he can be dangerous to Tyrion if he underestimates Varys.
Tyrion however, in his arrogance, believes that Varys is referring to him, being a "very small man" and begins considering him a friend.
This is the third and final lie of Varys' character that I wish to dissect here - the lie that he is a friend or useful ally of sorts to Tyrion.
The idea of Varys betraying Tyrion's trust and working against him is first presented to us by Cersei, when she lets slip that Varys informed her of Tyrion's plan to send the Hound into battle at the Battle of the Blackwater;
"Varys says so." The swan was too rich for his taste. A line appeared on Cersei's pale white brow, between those lovely eyes. "You put too much trust in that eunuch." "He serves me well." "Or so he'd have you believe. You think you're the only one he whispers secrets to? He gives each of us just enough to convince us that we'd be helpless without him. He played the same game with me, when I first wed Robert. For years, I was convinced I had no truer friend at court, but now . . ." She studied his face for a moment. "He says you mean to take the Hound from Joffrey." - ACOK - TYRION XII
On a first read, we are quick to dismiss Cersei's claims for how much she is built up as an antagonistic and untrustworthy character, but in later understanding her motives in her own POV chapters, we realise there may be an inkling of truth in her claims. In the case of the Hound, Varys has no reason to inform her of Tyrion's plan, however innocent and insignificant in the grand game of thrones they may be, other than to maintain some level of trust with her and to invoke conflict between the Lannister siblings, which benefits Varys.
There are frequent moments throughout the series in which Varys alludes to knowing key information and deliberately withholding it from Tyrion even if it may benefit Tyrion, including who ordered Mandon Moore to kill Tyrion;
Bronn had turned up all he could on Ser Mandon, but no doubt Varys knew a deal more . . . should he choose to share it. "The man seems to have been quite friendless," Tyrion said carefully. "Sadly," said Varys, "oh, sadly. You might find some kin if you turned over enough stones back in the Vale, but here . . . Lord Arryn brought him to King's Landing and Robert gave him his white cloak, but neither loved him much, I fear. Nor was he the sort the smallfolk cheer in tourneys, despite his undoubted prowess. Why, even his brothers of the Kingsguard never warmed to him. Ser Barristan was once heard to say that the man had no friend but his sword and no life but duty . . . but you know, I do not think Selmy meant it altogether as praise. Which is queer when you consider it, is it not? Those are the very qualities we seek in our Kingsguard, it could be said—men who live not for themselves, but for their king. By those lights, our brave Ser Mandon was the perfect white knight. And he died as a knight of the Kingsguard ought, with sword in hand, defending one of the king's own blood." The eunuch gave him a slimy smile and watched him sharply. Trying to murder one of the king's own blood, you mean. Tyrion wondered if Varys knew rather more than he was saying. - ASOS - TYRION II
When Tyrion presses Varys for information about Mandon, Varys deliberately skims over Mandon's life and offers vague beliefs about the perfect Kingsguard to try and derail Tyrion's line of questioning, and when Varys pretends not to have known about Mandon trying to kill Tyrion, he watches Tyrion "sharply" to try and gather how much Tyrion himself knows, whether or not Tyrion will seek retribution for it from the anger on his face, and almost openly mocks him for the smile he gives. Tyrion briefly considers the idea that Varys is deliberately holding information back from him, but chooses instead to talk about other matters, which is what Varys wanted.
On the matter of Varys' choosing to support the Lannisters or Stannis, Varys once again tries to confuse Tyrion with riddles and not directly answer the question;
"He accuses my brother and sister of incest. I wonder how he came by that suspicion." "Perhaps he read a book and looked at the color of a bastard's hair, as Ned Stark did, and Jon Arryn before him. Or perhaps someone whispered it in his ear." The eunuch's laugh was not his usual giggle, but deeper and more throaty. "Someone like you, perchance?" "Am I suspected? It was not me." "If it had been, would you admit it?" "No. But why should I betray a secret I have kept so long? It is one thing to deceive a king, and quite another to hide from the cricket in the rushes and the little bird in the chimney. Besides, the bastards were there for all to see." - ACOK - TYRION III
We know for certain that Varys didn't want the WotFK to start so soon or for the truth of Cersei's children to come out in AGOT because he wanted more time for Viserys' forces to grow in strength and come closer to Westeros in order to invade, either to take the throne for themselves or be the ones to kill the beloved Robert Baratheon and paint themselves as villainous usurpers to make fAegon more heroic and not taint him with Robert's blood on his hands.
However, like with Cersei and Tyrion, Varys needs to tell those on the Small Council a certain amount of secretive information in order to gain their trust and support, including Stannis. That Varys even chooses to give a deeper and more unsettling laugh about the matter is another example of him openly laughing about his treachery in Tyrion's face and Tyrion not realising it.
When Tyrion almost stumbles upon this ruse and Varys realises that his laugh was too deep and questionable, Varys tries to brush it off by saying its harder to keep a secret secret if so many people are investigating it, and insists that there were so many of Robert's bastards to find for Stannis to figure out the truth on his own. But again, this last statement from Varys is littered with half-truths.
Varys insists it is hard to keep deceiving King Robert about the truth of his children's true parentage, and even harder to keep deceiving the truth from those who are deliberately looking for the truth and wanting to find it. This in itself is a reference to his believe that Stannis was always power-hungry for the Iron Throne, and was just looking for an excuse to try to claim it for himself as Robert's true heir, regardless of whether or not Cersei's children were Robert's or not.
But his own comment about "betraying a secret I have held for so long" is a direct allusion to fAegon's existence, and his determination not to reveal the truth of his existence to anyone, including Tyrion.
Even when Tyrion rides with Illyrio and Jon Connington's entourage in ADWD, Tyrion is only led to believe that they are going to support Daenerys' claim for the Iron Throne, and has to work out for himself that fAegon is actually Aegon and the one Varys and Illyrio wish to crown;
Young Griff hesitated. "Lannister? Your father—" "—is dead. At my hand. If it please Your Grace to call me Yollo or Hugor, so be it, but know that I was born Tyrion of House Lannister, trueborn son of Tywin and Joanna, both of whom I slew. Men will tell you that I am a kingslayer, a kinslayer, and a liar, and all of that is true … but then, we are a company of liars, are we not? Take your feigned father. Griff, is it?" The dwarf sniggered. "You should thank the gods that Varys the Spider is a part of this plot of yours. Griff would not have fooled the cockless wonder for an instant, no more than it did me. No lord, my lordship says, no knight. And I'm no dwarf. Just saying a thing does not make it true. Who better to raise Prince Rhaegar's infant son than Prince Rhaegar's dear friend Jon Connington, once Lord of Griffin's Roost and Hand of the King?" "Be quiet." Griff's voice was uneasy. - ADWD - TYRION V
After Tyrion works out the truth and manipulates both fAegon and Jon Connington into riding for Westeros instead of joining forces with Daenerys, he disappears and goes off to seek her out.
This likely angers Varys, and is the reason why Varys no longer affectionately calls Tyrion by his name or by '"lord" and instead refers to him as "the imp" in ADWD's epilogue, a term that Varys knows Tyrion hates;
Ser Kevan tried to rise, but the strength had left him. He could not feel his legs. "I thought the crossbow fitting. You shared so much with Lord Tywin, why not that? Your niece will think the Tyrells had you murdered, mayhaps with the connivance of the Imp. The Tyrells will suspect her. Someone somewhere will find a way to blame the Dornishmen. Doubt, division, and mistrust will eat the very ground beneath your boy king, whilst Aegon raises his banner above Storm's End and the lords of the realm gather round him." "Aegon?" For a moment he did not understand. Then he remembered. A babe swaddled in a crimson cloak, the cloth stained with his blood and brains. "Dead. He's dead." - ADWD - EPILOGUE
Varys chooses to frame Tyrion for Kevan's murder, **deliberately and intentionally further putting his life in danger by Cersei's wrath, already knowing that Tyrion has a large bounty on his head because of her, and talks lowly of Tyrion by calling him an "Imp" and referring to his "connivance" in a very scorned tone.
Varys never considered Tyrion a friend or ally in ASOIAF and was just using him, another lie that he tells throughout the series, and by the end of ADWD, he hates Tyrion for his scheming and knowing too much information than Varys wished him to know.
But there is a bigger telling point about how much Varys uses Tyrion, and a more ground-breaking twist in the narrative that reflects both how much Varys underestimates Tyrion and how much Varys never cared for him;
In ASOS, Varys didn't tell Tyrion how to reach the Tower of the Hand to go and kill Tywin. Varys told him this key information so that Tyrion could go and kill Shae instead, who Varys considered a greater threat to him.
Shae is one of the few, perhaps the only, people in ASOIAF to see through Varys' disguises;
A whiff of something rank made him turn his head. Shae stood in the door behind him, dressed in the silvery robe he'd given her. I loved a maid as white as winter, with moonglow in her hair. Behind her stood one of the begging brothers, a portly man in filthy patched robes, his bare feet crusty with dirt, a bowl hung about his neck on a leather thong where a septon would have worn a crystal. The smell of him would have gagged a rat. "Lord Varys has come to see you," Shae announced. The begging brother blinked at her, astonished. Tyrion laughed. "To be sure. How is it you knew him when I did not?" She shrugged. "It's still him. Only dressed different." - ACOK - TYRION X
This catches Varys off guard, and any chance of seeing him being angry or looking threatened is waved away from the reader when Tyrion laughs it off and turns his attention again to his paramour Shae.
Wearing disguises is a very important part of Varys' many schemes, including visiting Ned Stark in the Black Cells as the gaoler Rugen;
"Wine," a voice answered. It was not the rat-faced man; this gaoler was stouter, shorter, though he wore the same leather half cape and spiked steel cap. "Drink, Lord Eddard." He thrust a wineskin into Ned's hands. The voice was strangely familiar, yet it took Ned Stark a moment to place it. "Varys?" he said groggily when it came. He touched the man's face. "I'm not … not dreaming this. You're here." The eunuch's plump cheeks were covered with a dark stubble of beard. Ned felt the coarse hair with his fingers. Varys had transformed himself into a grizzled turnkey, reeking of sweat and sour wine. "How did you … what sort of magician are you?" - AGOT - EDDARD XV
His disguise as Rugen is very important to Varys' plans after Tyrion's disappearance from King's Landing, as Varys seeks to frame Rugen as a Tyrell puppet who broke Tyrion from imprisonment on the Tyrells' behalf, to sow division and further paranoia between Cersei and the Tyrells;
Gold, yes, but the moment Cersei took it she could tell that it was wrong. Too small, she thought, too thin. The coin was old and worn. On one side was a king's face in profile, on the other side the imprint of a hand. "This is no dragon," she said. "No," Qyburn agreed. "It dates from before the Conquest, Your Grace. The king is Garth the Twelfth, and the hand is the sigil of House Gardener." Of Highgarden. Cersei closed her hand around the coin. What treachery is this? Mace Tyrell had been one of Tyrion's judges, and had called loudly for his death. Was that some ploy? Could he have been plotting with the Imp all the while, conspiring at Father's death? With Tywin Lannister in his grave, Lord Tyrell was an obvious choice to be King's Hand, but even so . . . "You will not speak of this with anyone," she commanded. - AFFC - CERSEI II
But this ploy to incite conflict between the Lannisters and Tyrells would have failed if Shae had lived, as she had seen through Varys' disguises and could've warned Cersei of it, something which Varys could not allow, especially given that he considered her one of Cersei's pawns after Tyrion's trial.
It is also fundamentally not in Varys' interest for Tyrion to kill Tywin, as keeping Tywin alive for now would ensure the Dornish would have someone to rise up and seek vengeance against for Elia's murder. As readers are shown in AFFC and ADWD Dornish chapters, the Martells are divided and frankly confused about how to seek vengeance for Elia's murder now that Tywin, Gregor and Armory Lorch - the three men involved in her murder - are now dead. **If Tywin had lived, Varys could've played on the Dornish thirst for vengeance against him and had fAegon promise them it in exchange for their support, something which they likely would've agreed to.
So by this point in the post, we've explored some of the key lies of Varys' character to know for sure what he is not;
So who is the real Varys? Behind all of these lies and deceptions, who is he really deep down?
Well, Varys is not a eunuch, and in fact fAegon's true father by blood. Varys has deliberately spent his life lying about being a eunuch in order to discourage people from the truth that he is the father of fAegon by Illyrio’s sister, and wants to control the Iron Throne through fAegon.
Recall Varys' conversation with Tyrion in ACOK that I previously cited earlier;
"He accuses my brother and sister of incest. I wonder how he came by that suspicion." "Perhaps he read a book and looked at the color of a bastard's hair, as Ned Stark did, and Jon Arryn before him. Or perhaps someone whispered it in his ear." The eunuch's laugh was not his usual giggle, but deeper and more throaty. "Someone like you, perchance?" "Am I suspected? It was not me." "If it had been, would you admit it?" "No. But why should I betray a secret I have kept so long? It is one thing to deceive a king, and quite another to hide from the cricket in the rushes and the little bird in the chimney. Besides, the bastards were there for all to see." - ACOK - TYRION III
"Why should I betray a secret I have kept so long?" is the biggest hint towards fAegon's existence and Varys' own true motivations in the entire series. Varys makes this spur of the moment claim after being caught off guard by Tyrion and doesn't think it through before saying it and letting Tyrion realise there is more to him.
Varys was never castrated and he is not bald. He deliberately shaves his head to hide his true parentage and lies about being a eunuch to discourage people from believing that he is capable of fathering a child, a child with Blackfyre blood that he wishes to sit on the Iron Throne, a secret that Varys has cared for and kept so long.
The last time we see Varys in ADWD, he finally begins to tell the truth about fAegon's existence, and when he does, and speaks about the son he loves, he begins to speak in his real, deeper and non-emasculated voice;**
"Aegon?" For a moment he did not understand. Then he remembered. A babe swaddled in a crimson cloak, the cloth stained with his blood and brains. "Dead. He's dead." "No." The eunuch's voice seemed deeper. "He is here. Aegon has been shaped for rule since before he could walk. He has been trained in arms, as befits a knight to be, but that was not the end of his education. He reads and writes, he speaks several tongues, he has studied history and law and poetry. A septa has instructed him in the mysteries of the Faith since he was old enough to understand them. He has lived with fisherfolk, worked with his hands, swum in rivers and mended nets and learned to wash his own clothes at need. He can fish and cook and bind up a wound, he knows what it is like to be hungry, to be hunted, to be afraid. Tommen has been taught that kingship is his right. Aegon knows that kingship is his duty, that a king must put his people first, and live and rule for them." - ADWD - EPILOGUE
This is who Varys truly is - not a eunuch, not a victim of slavery or castration, not a man who weeps for the suffering of children, not a friend of Tyrion Lannister, but a loving father who wishes to cast a very large shadow over the greatest empire the world of ASOIAF has ever known.
TLDR:
Varys is not a eunuch but instead the father of fAegon, a secret he has spent his life protecting, and a lie he has spent his life telling to better protect his son.
Varys does not hate R'hllor or those who worship magical flames, his entire story about his manhood being sacrificed to flames is a lie he tells to gain Tyrion's trust in their efforts to defeat Stannis in ACOK. Varys is close friends with Illyrio, a man who praises R'hllor, and spent the early parts of his life encouraging Aerys to feed innocent people to the flames of wildfire.
Varys does not hate the suffering of innocent children, he was prepared to buy an innocent child from a man and sacrifice him in order to protect the "real" Aegon, he was prepared to abandon Rhaenys to her brutal death even if he could have saved her too and prepared to send Viserys and Daenerys into a doomed invasion of Westeros that would've led to their deaths just to make fAegon appear a more heroic saviour compared to them.
Varys was never a friend or ally to Tyrion but instead was using him ever since they first met. He has repeatedly withheld important information from him such as who hired Mandon Moore to kill Tyrion and how Stannis figured out the truth about Cersei's children, and on frequent occasions has nearly openly laughed in Tyrion's face at both his misfortune and the information Varys withheld from him. Varys also manipulated Tyrion into killing Shae so that she would not expose Varys' identity as Rugen and didn't trust Tyrion with the information about fAegon's existence, something that Tyrion had to figure out himself. Varys also talks lowly of Tyrion at the end of ADWD as a "conniving Imp" which reflects Varys' true feelings about Tyrion.
Varys has spent his life shaving his hair and lying about being a eunuch to discourage anyone from believing he could father fAegon and better protect him, so that fAegon could one day take the Iron Throne, and cast a very large shadow over the Targaryen Empire forever.

This is the argument many People put forward when the issue of Love Jihad comes up. Of course, Hindu men commit crimes against Hindu women, just like Muslim men commit crimes against Muslim women, Sikh men against Sikh women, Christian men against Christian women and so on. These crimes are of various nature, some very very serious. (For that matter women also commit crimes against men, but it is out of scope of this write up).
As a society we have acknowledged these crimes and made laws, often strict, to prevent them. As these crimes are often social in nature, only law is not sufficient to deal with them, so we also teach our young and try to mold the society in the desired manner. Having acknowledged these crimes exist,
lets now see why these crimes are different from what is popularly called “Love Jihad”.
1 Hate crime : A crime is considered a hate crime if the victim was targeted based on their identity. For example, muggings happen in New York subway, but if someone mugs only Jewish people, then it is not just a mugging, it is a hate crime and the criminal is charged with stricter laws. The same applies to Love Jihad cases where Hindu women are specifically targeted. In Ajmer sex scandal, out of the 250 victims, how many were Muslim? It was hate crime, not a social crime. Clubbing them with regular crimes is deliberate obfuscation to lower the seriousness of these crimes.
2 Based in Islamic doctrine : Islam treats Kafir and Muslim women differently in every aspect. For example, Kafir women can be kept as slaves but can not be married. On the other hand, you can not enslave a Muslim woman. These attitudes are extremely misogynistic and kafir-phobic which lead to many of these crimes. When your ideology dehumanizes an entire group of people, your ideology is encouraging crimes against that group. Hence Abduls feel no shame in hiding their identity or forcing Hindu women to convert and worse. This is not a regular crime, this is religious.
3 Backed by the wider society : As seen in the recent case in Pune, when a Hindu underage girl is abducted the entire Muslim society seems to hush it up. The mother of the accused, the Maulvi who converts and marries the girl knowing fully well she is underage, the family, even the first wife or wives fully support these criminal acts. Ever heard a Muslim whistle-blower saving a Hindu girl? This points to a very unhealthy mindset that is different from regular crimes. This makes saving these girls far far more difficult.
4 Repeating patterns : Many patterns repeat in these cases. It may appear simplistic, but faking identity, use of force and intimidation, targeting underage girls, conversion, involvement of more than just one person, violence etc repeat in many cases. Not to deny other complexities, but this obvious presence of recurring patterns itself makes these crimes different from other crimes. The reason we are discussing crime is to prevent it right? Then failing to identify patterns will only lead to failure to prevent such crimes.
5 Organized apologia : A tell tale clue is the existence of organized intellectual class that keeps denying love jihad exists. No such gangs exist to deny regular crimes. On the other hand popular media keeps glorifying relations between Hindu girls and Muslim men, but not the other way around. How many Feminism and Youth forums on Instagram have covered the 100s of attacks on Hindu men for talking to Muslim women? None. The very existence of such people in media, academia, entertainment etc reveals that the nature of these crimes is indeed different.
6 : Disturbing no of underage cases : A truly disgusting aspect that is common is the shocking no of underage girls being groomed by older men. If you look at the recent cases from Delhi, Uttarakhand, Pune etc, ALL of them involve underage girls incapable of consent. This phenomenon is the clearest evidence of organised grooming. Islamic doctrine permits relations with underage girls as long as they have hit puberty, but very young girls are also easy to brainwash and turn against their families and society. Anyone calling this organized pedophilia as “just interfaith love” is part of this pedophilia grooming gang, nothing less. So next time anyone says “But Hindu men also commit crimes against Hindu women”, tell them they may or may not know it, but they are part of a grooming gang and they should be ashamed of themselves.

Edit: This is also a DISCUSSION which means i would be very happy to hear your thougts or corrections. If you encounter an error or atleast a disagreement, please voice them. Negative or positive i love to hear them.
Kokomi is proven to be a very capable military leader and tactician. She had fought a long and destructive civil war of which she had inferior numbers, inferior positioning, a naval blockade, and not to mention a disadvantageous economic position. She fought her battles in inhospitable places like Yashiori island of which was cursed by the Shogun with everlasting lightning before us the players stopped it. So what really made Kokomi's tactics succeed over her enemies?
To understand Kokomi's tactics we must understand her philosophies on war. Distinctively her means of warfare is heavily drawn upon or atleast similar to "The Art of War" by Sun Tzu or atleast its teyvat counterparts(*This is not a joke, the trailer litterally referenced it in a bit cringy way imo). This philosophy of war rest in the belief that Victory ought to be achieved in the minimum of casualties and the maximum of gain. In the trailer kokomi's strategizing hinted a bit at this. When she sent two of her soldier groups to battle while her personal professional force, the swordfish would burn the provisions. The act of burning provisions could force the enemy to retreat even after a winning fight, therefore minimizing casualties for both sides. Yet war philosophies alone won't win wars, afterall the devil is in the details.
The most important key lies in the tip of the spear. The Sangonomiyan Armed Forces. There is nothing special about individual samurais on the army but maybe perhaps that's why it is special. The sangonomiyan army is actually implied to be more ragtag than the Shogun's. Limited supplies with a strained food supply chain, many of them are implied on being illiterate(*), they could only be implied as either as trained or less trained as their inazuman counterparts. Not to mention the rebels diversity as they came from many parts of inazuma and not just watatsumi. They have been proven to be somewhat easily agitated indicated by the diplomatic incident of one accusing the inazuman armed forces of colluting with the fatui, if the soldiers have lied about their non involvement they could just get away with it and come back with better terms, not to mention when some of them almost mutinied because they wanted more fighting. These are inherently bad traits for the army, so why does it become a very effective force under kokomi?
Usually if you like roaming into youtube about wars and setpiece battles you would see a bird's eye view on the battle taking place. Intuitively from my prespective and experiences this had led me to believe that troop placement and clever use of terrain is the key to winning a battle, that is true however, it does not paint a full picture in winning wars. The romans weren't effective just because Caesar put a hidden infantry detachment behind his cavalry, or scipio's copy of Hannibal's strong wings weak center. A real life example of this is the battle of Munda, Caesar was at his most disadvantageous position, fighting an uphill battle against a numerically superior enemy. Yet he won, this shows that battle positioning is not the only way a battle or war is won.
One of the most important thing that The Sangonomiyan armed forces have and its most crucial aspect is Divisional structures. Though a bit counterintuitive, this part is actually crucial. Kokomi divide her soldiers into platoons, in the trailer we are hinted at the names of these platoons or atleast their leaders, Gobius and Gorgasia. Divisional structures ensure that there is cohesion within the lines and a clear structure of power. Orders can be carried out easier and battle lines could be easily drawn, assessed and fortified or enforced. Part of the reason why the romans were so effective is that they have cohesive structures, albeit copied from other places. This help them identify which section og the line is weakest and help some to enforce a position and make orders more effective as the division of labor and duties could be easily assigned. This is examplified in a real life example where Crassus' son helped reinforced a section of Caesar line in the left flank in the war against Ariovistus. In the trailer kokomi is shown to be taking this to her advantage. Gobius and Gorgasia was tasked to lure the enemy into a trapped by a feigned retreat(*) while her specialized platoon called swordfish are tasked to burn provisions. This level of management is crucial in decision making and effectiveness. Yet the strenght of an army is not merely of its structure but how we use it.
In examining Kokomi's way of warmaking and strategizing, i was somewhat reminded of Prussia's millitary reform after Napoleon's invasion. Several keys are fast mobilization, flexible leadership, and decentralized yet coordinated forms of organization. Kokomi is shown to trust her subordinates, this high levels of trust shows the effectiveness of her general staff. We saw how some forces could patrol indepent from HQ, and some are tasked with missions without Kokomi's direct intervention. This is what is called a mission type tactic, where a commander are given broad command over the field but subordinates are given the freedom for initiative. The orders on these type of tactic is a bit more vague therefore letting the platoon commanders figure out the way with their own experiences. This is the tactic of "No plans survive first contact with the enemy" type of deal. This made command flexible, and attacks more swift. This also makes it easier to imply the tactic of March Divided fight united which will makes troops moves faster, but by far i haven't seen it in action in game. What is in game is a mission where we are tasked to clear up a section of watatsumi island, where we fight monsters, part of the platoons are already fighting there before we intervened, this platoon was a bit away from gorou or kokomi.
Her strict rules and the fact that most remembers them or atleast her higher ups means there is a form of discipline within the ranks. Even with the diversity all is united in a goal of defeating the shogun which makes morale high, one personnel even tried drastic meassures in obtaining an illusion and using it though it is a tragedy what happened to him. The cohesion in her army despite the shortcomings ran contrast to Kujou Sara's army which has an implied low morale and some disubordination, the fact that some of the army colluted with the fatui shows relevant doubt in the ranks. This paired with kokomi's excellent judgement of timing and terrain, shown in her setpiece battle with kujou sara, the first and only battle we saw at Nazuichi beach, how she outflanked and outmanuvered Sara's forces in the last minute, where she pushed with all her infantry (massive L for Sara, a good general would have reserves to protect the flanks).
Kokomi's leadership is also very well received, she has the very trust of her people and my heart. Even when she's a cute anxious introvert who my balls itch for, she could balance it with significant degree of profesionalism. She is still very young, and had a lot to learn like governing, but with time she will learn it through experience. There is still probably a lot i wanted to say in here but, well would you really want to read a very long post about how to improve watatsumi island? This is probably a love letter from me to Kokomi, as a lover of history and wargaming, as an aut- (my lawyer has informed me to not do this joke) who plays imperator rome, victoria 3 and Hoi4, i love you kokomi

Let's then talk about training. It is a bit implied that some of the soldiers can't read which is normal for an army yes. My question, where be them standardized training yo? Well there is a segment where we train with them but... not sure how shooting an arrow would help... in this endeavor. You want standardized basic training. Mass scale, all encompassing basic training. You gotta train the muscles. A roman legion took months to train and not all can qualify. The formation in BOTH of the armies are a laughing stock. S1: Carolann. Now I get that Carolann didn't have that team spirit and that caused her to be eliminated but I mean when we get later in the season Michael, Ralph and Jessica weren't showing that team spirit when they let Elsie sink and plus throughout HK history there was worse team players in HK that stayed a whole lot longer
S2: Keith. I get that his pass performance was the worst out of the final 3 but I mean come on man Keith led his blue team throughout the season and only to lose to Virginia who only had 1 good service and was up for elimination like 5 times is ridiculous
S3: Brad. I get that Brad had his inconsistencies but he's a lot of good things while Josh was consistently horrible and that's all I'm gonna say
S4: Louross. Louross, like Brad had his inconsistencies but did quite a bit of good and he knows how to motivate a team but Matt had that one good service and ran with that and I get that Louross's final service was bad but Matt's final service before Black Jackets was literally no better and Shayna low-key got robbed, even though Shayna was pretty forgettable and wasn't winning anyways. Rosann was a service fucking trainwreck and really overstayed her place
S5: Actually I think every elimination was fair in this season prolly not Ji bc of injury but she left on her own. You can argue Giovanni leaving over Andrea bc he was better than her overall but the talking and the burning Robert with the pan really killed him. Now I think Ramsey would let Giovanni slide with the talking back but when burned Robert with a hot pan in the fridge without telling nobody, that was a level of incompetence that Ramsey couldn't handle anymore. So I think S5 was pretty fair
S6: I honestly think that Jim leaving over Andy was kinda unfair. I get that Jim lacked passion and energy, Andy was getting fucking battered all season nonstop bc of consistent inconsistencies
S7: Like S5 I think S7 was pretty fair when it came to eliminations. Like did Fran and Scott overstay their welcome? Yes, but I don't think they survived over chefs that deserved to stay. You can argue Jason getting eliminated over Benjamin at the final 5 bc Benjamin did have a bad service but overall he was probably better than Jason but they both had really good runs in HK
S8: I hear a lot of people say that Vinny was robbed of a Black Jacket from Sabrina and I honestly don't agree bc Vinny had 3 terrible services back to back to back. So I'm gonna say Curtis just bc they put him on a station they knew he'd struggle on but Curtis wasn't gonna last much longer
S9: I'm gonna say Elizabeth for this one. Now she was not a big personality and she wasn't going to win due to her lack of leadership and confidence but she was a very consistent service performer and Elise staying over her was pretty BS
S10: I'm going to pick Brian. Like Brad and Louross his inconsistencies were starting to show by the end but he was a pretty strong member of the Blue Team and Robyn had so many chances that not a lot of people would've had
S11: Easily Anthony. I mean him, Jon and Zach tried to carry the Blue team as much as they could and then when Zach started to go down hard-core, Jon and Anthony held up their part. But then Anthony had ONE BAD SERVICE he was eliminated over Zach even though Ramsey had to cook food on his station but Anthony still goes home and honestly Susan overstayed her welcome also. Anthony deserved that Black Jacket over Susan
S12: I think this season was also very fair when it comes to eliminations. You could say that a couple of DeMarco's nominations are ridiculous but ehhh.
S13: Look I love Sterling, you love Sterling, we all love Sterling but Fernando deserved to stay over Sterling and if not over Sterling, Fernando deserved to stay over Roe, either one of the 2 and Ashley getting eliminated was kinda lame also
S14: Alison getting eliminated over Josh. While Josh showed a lot of talent, he was getting put up for elimination too many times and while Alison's last service was really bad I felt like she deserved another chance
S15: I've been waiting to speak on this one. Jackie had way too many chances for someone who sucked. While Jackie staying over Chad, Jackie staying over Joe and Jackie staying over Hassan were all unfair, the most unfair was Jackie staying over Eddie. I mean the fact that Eddie got eliminated was eliminated bc he was late on garnish for 15 seconds for ONE TABLE will never sit right with me
S16: Oh boy this season. Johnny staying over Aaron was BS. I get Aaron lacked confidence and a voice but he didn't do anything wrong to get eliminated this service. Also Paulie staying over Koop, while both had their downs I felt that Koop was a bit better than Paulie and Paulie staying over Devin I mean Devins services were better than Paulies
S17: Oh My God. More egregious than S16. Alright off rip Elise and Barbie staying over Van was some nonsense, Robyn staying over Giovanni was lame, Robyn getting a higher placement over Dana was shameful and Robyn getting a Black Jacket over Jennifer was disgraceful. But everyone knows that Nick getting robbed of the victory almost Ruined HK for a lot of people like his growth from S14 to S17 was so immense that he was a frontrunner from beginning to end. He was a better leader in S17 than he was in S14, he was more of a consistent chef in S17 than he was in S14 and to go through all that just to lose bc of a CEO, not even an actual CHEF? BULLSHIT!!! Like Nick had the skill, the leadership and he was in the prime of his life at 28 years old just to lose in the most lame way possible and I can't accept that
S18: Kanae getting eliminated over Heather was WACK ASF. Heather was really starting to struggle by the end and her performance at the Black Jacket Challenge was pretty bad but Kanae black jacket challenge performance was good but there was dishes that were better than hers and Heather's final dish was good but she played it safe but Kanae's final dish was also good but she took a chance and I think Kanae deserved that
S19: Now while Adam getting eliminated over Marc was WEAK. My pick is Nikki getting eliminated over Amber bc Nikki started off slow she really improved after Ep.2 like she had so many standout services and challenges like I thought she was gonna get that jacket and while Amber started out strong, her flaws really started to show when she got to the blue team and flat out stole that jacket from Nikki plain and simple
S20: Morgana getting eliminated over Payton was kinda unfair. Her lack of confidence and voice really failed her but Payton over all was a way weaker chef. But my pick for the most unfair of this season was Keanu getting eliminated in that challenge bc she was so so so strong up to that point but she got eliminated over that burnt pork chop but I think she was the strongest chef that had to do that challenge but she got eliminated in lame fashion
S21: I think these eliminations were pretty fair this season also. I mean you could probably argue Brett staying over Abe but that's probably or most likely Cheyenne leaving over Dafne but it could've been either one of them so it's a toss up

Pain Management Best Practices Inter-Agency Task Force - Draft Report on Pain Management Best Practices: Updates, Gaps, Inconsistencies, and Recommendations Official Health and Human Services Department Released December 2018
“The Comprehensive Addiction and Recovery Act (CARA) of 2016 led to the creation of the Pain Management Best Practices Inter-Agency Task Force (Task Force), whose mission is to determine whether gaps in or inconsistencies between best practices for acute and chronic pain management exist and to propose updates and recommendations to those best practices. The Task Force consists of 29 experts who have significant experience across the disciplines of pain management, patient advocacy, substance use disorders, mental health, and minority health.”
In addition to identifying approximately 60 gaps in clinical best practices and the current treatment of pain in the United States, HHS PMTF provided recommendations for each of these major areas of concern. In alignment with their original charter, the PMTF will submit these recommendations to Congress to become our ‘National Pain Policy’. The 60+ gaps and inconsistencies with their recommendations will serve to fill gaps in pain treatment at both the state and federal level; and the overwhelming consensus was that the treatment of pain should be multimodal and completely individualized based on the individual patient. The heart of each recommendation in each section was a resounding call for individualization for each patient, in regards to both non-pharmacological and pharmacological modalities; including individualizations in both opioid and non-opioid pharmacological treatments.
While each of the gap+recommendation sections of what is poised to become our national pain policy is extremely important, one that stands out the most (in regards to opioid prescribing) is the Stigma section. Contained in this section is one of the core statements that shows our Health and Human Services agency - the one that should have always been looked to and followed - knew the true depth of the relationship (or lack of) between the overdose crisis and compassionate prescribing to patients with painful conditions:
“The national crisis of illicit drug use, with overdose deaths, is confused with appropriate therapy for patients who are being treated for pain. This confusion has created a stigma that contributes to raise barriers to proper access to care.”
The recommendation that follows - “Identify strategies to reduce stigma in opioid use so that it is never a barrier to patients receiving appropriate treatment, with all cautions and considerations for the management of their chronic pain conditions” - illustrates an acknowledgment by the top health agency of the federal government that the current national narrative conflating and confusing compassionate treatment of pain with illicit drug use, addiction, and overdose death is incorrect and only serving to harm patients.
Since March of 2016 when the CDC Guidelines were released, advocates, patients, clinicians, stakeholders, and others, have began pointing out limitations and unintended consequences as they emerged. In order to address the unintended consequences emerging from the CDC Guidelines, this task force was also charged with review of these guidelines; from expert selection, evidence selection, creation, and continuing to current misapplication in order to provide recommendations to begin to remedy these issues.
“A commentary by Busse et al. identified several limitations to the CDC guideline related to expert selection, evidence inclusion criteria, method of evidence quality grading, support of recommendations with low-quality evidence, and instances of vague recommendations. In addition, the CDC used the criterion of a lack of clinical trials with a duration of one year or longer as lack of evidence for the clinical effectiveness of opioids, whereas Tayeb et al. found that that was true for all common medication and behavioral therapy studies.
Interpretation of the guideline, in addition to some gaps in the guideline, have led to unintended consequences, some of which are the result of misapplication or misinterpretation of the CDC guideline.
However, at least 28 states have enacted legislation related to opioid prescription limits, and many states and organizations have implemented the guideline without recognizing that the intended audience was PCPs; have used legislation for what should be medical decision making by healthcare professionals; and have applied them to all physicians, dentists, NPs, and PAs, including pain specialists.441–444 Some stakeholders have interpreted the guideline as intended to broadly reduce the amount of opioids prescribed for treating pain; some experts have noted that the guideline emphasizes the risk of opioids while minimizing the benefit of this medication class when properly managed.”

The Pain Management Task Force addressed 8 areas that are in need of update or expansion with recommendations to begin remediation for each problem area:
Lack of high-quality data exists for duration of effectiveness of opioids for chronic pain; this has been interpreted as a lack of benefit Conduct studies Focus on patient variability and response for effectiveness of opioids; use real-world applicable trials
Absence of criteria for identifying patients for whom opioids make up significant part of their pain treatment Conduct clinical trials and/or reviews to identify sub-populations of patients where long-term opioid treatment is appropriate
Wide variation in factors that affect optimal dose of opioids Consider patient variables for opioid therapy: Respiratory compromise Patient metabolic variables Differences in opioid medications/plasma concentrations Preform comprehensive initial assessment it’s understanding of need for comprehensive reevaluations to adjust dose Give careful considerations to patients on opioid pain regimen with additional risk factors for OUD

“If harms to chronic pain patients are an unintended consequence of policies to reduce inappropriate prescribing, the government should seek to immediately minimize and measure the negative impacts of these policies. Any response should avoid further stigmatizing chronic pain patients, who are increasingly associated with — and sometimes blamed for — the overdose crisis and characterized as “drug seekers,” rather than people with serious health problems that require treatment.
Top government officials, including the President, have said the country should aim for drastic cutbacks in prescribing. State legislatures encourage restrictions on prescribing through new legislation or regulations. The Drug Enforcement Administration (DEA) has investigated medical practitioners accused of overprescribing or fraudulent practice. State health agencies and insurance companies routinely warn physicians who prescribe more opioids than their peers and encourage them to reduce prescribing. Opioid Prescribing Workgroup December 2018
This is material from the Board of Scientific Counselors in regards to their December 12, 2018 meeting that culminated the works of a project titled the “Opioid Prescribing Estimates Project.” This project is a descriptive study that is examining opioid prescribing patterns at a population level. Pain management is a very individualized process that belongs with the patient and provider. The Workgroup reviewed work done by CDC and provided additional recommendations.
SUMMARY There were several recurrent themes throughout the sessions.
Repeated concern was voiced from many Workgroup members that the CDC may not be able to prevent conclusions from this research (i.e. the benchmarks, developed from limited data) from being used by states or payors or clinical care systems to constrain clinical care or as pay-for- performance standards – i.e. interpreted as “guidelines”. This issue was raised by several members on each of the four calls, raising the possibility that providers or clinical systems could thus be incentivized against caring for patients requiring above average amounts of opioid medication.
Risk for misuse of the analysis. Several members expressed concerns that this analysis could be interpreted as guidance by regulators, health plans, or clinical care systems. Even though the CDC does not plan to issue this as a guideline, but instead as research, payors and clinical care systems searching for ways to reign in opioid prescribing may utilize CDC “benchmarks” to establish pay-for-performance or other means to limit opioid prescribing. Such uses of this work could have the unintended effect of incentivizing providers against caring for patients reliant upon opioids.
…It was also noted that, in order to obtain sufficient granularity to establish the need for, dosage, and duration of opioid therapy, it would be necessary to have much more extensive electronic medical record data. In addition, pain and functional outcomes are absent from the dataset, but were felt to be important when considering risk and benefit of opioids.

This issue was felt to be particularly challenging for patients on chronic opioids (i.e. “legacy” patients). In addition, the importance of measuring risk and benefit of tapering was noted. Not all high-dose patient populations benefit from tapering.
Post-Surgical Pain
General comments. Workgroup members noted that most patients prescribed opioids do not experience adverse events, including use disorder. Many suggested that further discussion of opioids with patients prior to surgery was important, with an emphasis on expectations and duration of treatment. A member suggested that take-back programs would be more effective than prescribing restrictions.
Procedure-related care. Members noted that patient factors may drive opioid need more than characteristics of a procedure.
Patient-level factors. Members noted that opioid-experienced patients should be considered differently from opioid-inexperienced patients, due to tolerance.
Chronic Pain
It was noted that anything coming out of the CDC might be considered as guidelines and that this misinterpretation can be difficult to counter. There was extensive discussion of the 50 and 90 MME levels included in the CDC Guidelines. It was recommended that the CDC look into the adverse effects of opioid tapering and discontinuation, such as illicit opioid use, acute care utilization, dropping out of care, and suicide. It was also noted that there are major gaps in guidelines for legacy patients, patients with multiple diagnoses, pediatric and geriatric patients, and patients transitioning to lower doses.
There were concerns that insufficient clinical data will be available from the dataset to appropriately consider the individual-level factors that weigh into determination of opioid therapy. The data would also fail to account for the shared decision-making process involved in opioid prescribing for chronic pain conditions, which may be dependent on primary care providers as well as ancillary care providers (e.g. physical therapists, psychologists, etc).
Patient-level factors. Members repeatedly noted that opioid-experienced patients should be considered differently from opioid-experienced patients, due to tolerance.
Members noted that the current CDC guidelines have been used by states, insurance companies, and some clinical care systems in ways that were not intended by the CDC, resulting in cases of and the perception of patient abandonment. One option raised in this context was to exclude patients on high doses of opioids, as those individuals would be qualitatively different from others. A variant of this concern was about management of “legacy” patients who are inherited on high doses of opioids. Members voiced concerns that results of this work has caused harm to patients currently reliant upon opioids prescribed by their providers.
Acute Non-Surgical Pain
Patient-level factors. Members felt that opioid naïve versus experienced patients might again be considered separately, as opioid requirements among those experienced could vary widely.
...Guidelines were also noted to be often based on consensus, which may be incorrect.
Cancer-Related and Palliative Care Pain
It was noted that the CDC guidelines have been misinterpreted to create a limit to the dose of opioids that can be provided to people at all stages of cancer and its treatment. It was also noted that the cancer field is rapidly evolving, with immunotherapy, CAR-T, and other novel treatments that affect response rates and limit our ability to rely upon historical data in establishing opioid prescribing benchmarks.
Concern that data would not be able to identify all of the conditions responsible for pain in a patient with a history of cancer (e.g. people who survive cancer but with severe residual pain). Further, it was noted that certain complications of cancer and cancer treatment may require the least restrictive long-term therapy with opioids.
The definition of palliative care was also complicated and it was suggested that this include patients with life-limiting conditions.
Overall, it was felt that in patients who may not have long to live, and/or for whom returning to work is not a possibility, higher doses of opioids may be warranted.

Dear Ms. Villar:
We are a group of scientists at CDC that are very concerned about the current state of ethics at our agency. It appears that our mission is being influenced and shaped by outside parties and rogue interests. It seems that our mission and Congressional intent for our agency is being circumvented by some of our leaders. What concerns us most, is that it is becoming the norm and not the rare exception. Some senior management officials at CDC are clearly aware and even condone these behaviors. Others see it and turn the other way. Some staff are intimidated and pressed to do things they know are not right. We have representatives from across the agency that witness this unacceptable behavior. It occurs at all levels and in all of our respective units. These questionable and unethical practices threaten to undermine our credibility and reputation as a trusted leader in public health. We would like to see high ethical standards and thoughtful, responsible management restored at CDC. We are asking that you do your part to help clean up this house!
It is puzzling to read about transgressions in national media outlets like USA Today, The Huffington Post and The Hill. It is equally puzzling that nothing has changed here at CDC as a result. It’s business as usual. The litany of issues detailed over the summer are of particular concern:
Recently, the National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) has been implicated in a “cover up” of inaccurate screening data for the Wise Woman (WW) Program. There was a coordinated effort by that Center to “bury” the fact that screening numbers for the WW program were misrepresented in documents sent to Congress; screening numbers for 2014 and 2015 did not meet expectations despite a multi­million dollar investment; and definitions were changed and data “cooked” to make the results look better than they were. Data were clearly manipulated in irregular ways. An “internal review” that involved staff across CDC occurred and its findings were essentially suppressed so media and/or Congressional staff would not become aware of the problems. Now that both the media and Congresswoman DeLauro are aware of these issues, CDC staff have gone out of their way to delay FOIAs and obstruct any inquiry. Shouldn’t NCCDPHP come clean and stop playing games? Would the ethical thing be to answer the questions fully and honestly. The public should know the true results of what they paid for, shouldn’t they?
Another troubling issue at the NCCDPHP are the adventures of Drs. Barbara Bowman and Michael Pratt (also detailed in national media outlets). Both seemed to have irregular (if not questionable) relationships with Coca­Cola and ILSI representatives. Neither of these relationships were necessary (or appropriate) to uphold our mission. Neither organization added any value to the good work and science already underway at CDC. In fact, these ties have now called into question and undermined CDC’s work. A cloud has been cast over the ethical and excellent work of scientists due to this wanton behavior. Was cultivating these relationships worth dragging CDC through the mud? Did Drs. Bowman and Pratt have permission to pursue these relationships from their supervisor Dr. Ursula Bauer? Did they seek and receive approval of these outside activities? CDC has a process by which such things should be vetted and reported in an ethics review, tracking and approval system (EPATS). Furthermore, did they disclose these conflicts of interest on their yearly OGE 450 filing. Is there an approved HHS 520, HHS 521 or “Request for Official Duty Activities Involving an Outside Organization” approved by Dr. Bauer or her Deputy Director Ms. Dana Shelton? An August 28, 2016 item in The Hill details these issues and others related to Dr. Pratt.
It appears to us that something very strange is going on with Dr. Pratt. He is an active duty Commissioned Corps Officer in the USPHS, yet he was “assigned to” Emory University for a quite some time. How and under what authority was this done? Did Emory University pay his salary under the terms of an IPA? Did he seek and receive an outside activity approval through EPATS and work at Emory on Annual Leave? Formal supervisor endorsement and approval (from Dr. Bauer or Ms. Shelton) is required whether done as an official duty or outside activity.
If deemed official, did he file a “Request for Official Duty Activities Involving an Outside Organization” in EPATS? Apparently Dr. Pratt’s position at Emory University has ended and he has accepted another position at the University of California ­ San Diego? Again, how is this possible while he is still an active duty USPHS Officer. Did he retire and leave government service? Is UCSD paying for his time via an IPA? Does he have an outside activity approval to do this? Will this be done during duty hours? It is rumored that Dr. Pratt will occupy this position while on Annual Leave? Really? Will Dr. Pratt be spending time in Atlanta when not on Annual Leave? Will he make an appearance at NCCDPHP (where he hasn’t been seen for months). Most staff do not enjoy such unique positions supported and approved by a Center Director (Dr. Bauer). Dr. Pratt has scored a sweet deal (not available to most other scientists at CDC). Concerns about these two positions and others were recently described in The Huffington Post and The Hill. His behavior and that of management surrounding this is very troubling.
Finally, most of the scientists at CDC operate with the utmost integrity and ethics. However, this “climate of disregard” puts many of us in difficult positions. We are often directed to do things we know are not right. For example, Congress has made it very clear that domestic funding for NCCDPHP (and other CIOs) should be used for domestic work and that the bulk of NCCDPHP funding should be allocated to program (not research). If this is the case, why then is NCCDPHP taking domestic staff resources away from domestic priorities to work on global health issues? Why in FY17 is NCCDPHP diverting money away from program priorities that directly benefit the public to support an expensive research FOA that may not yield anything that benefits the public? These actions do not serve the public well. Why is nothing being done to address these problems? Why has the CDC OD turned a blind eye to these things. The lack of respect for science and scientists that support CDC’s legacy is astonishing.
Please do the right thing. Please be an agent of change.

There is no question that there is an opioid misuse epidemic and that efforts need to be made to control it. The Centers for Disease Control and Prevention (CDC) is applauded for its steps to undertake this lofty effort. However, based on the American Academy of Family Physicians’ (AAFP’s) review of the guideline, it is apparent that the presented recommendations are not graded at a level consistent with currently available evidence. The AAFP certainly wants to promote safe and appropriate prescribing of opioids; however, we recommend that the CDC still adhere to the rigorous standards for reliable and trustworthy guidelines set forth by the Institute of Medicine (IOM). The AAFP believes that giving a strong recommendation derived from generalizations based on consensus expert opinion does not adhere to evidence-based standards for developing clinical guideline recommendations.
The AAFP’s specific concerns with the CDC’s methodology, evidence base, and recommendations are outlined below.
Methodology and Evidence Base
All of the recommendations are based on low or very low quality evidence, yet all but one are Category A (or strong) recommendations. The guideline states that in the GRADE methodology "a particular quality of evidence does not necessarily imply a particular strength of recommendation." While this is true, it applies when benefits significantly outweigh harms (or vice versa). When there is insufficient evidence to determine the benefits and harms of a recommendation, that determination should not be made.
When evaluating the benefits of opioids, the evidence review only included studies with outcomes of at least one year. However, studies with shorter intervals were allowed for analysis of the benefits of nonopioid treatments. The guideline states that no evidence shows long-term benefit of opioid use (because there are few studies), yet the guideline reports "extensive evidence" of potential harms, even though these studies were of low quality. The accompanying text also states "extensive evidence" of the benefits of non-opioid treatments, yet this evidence was from shorter term studies, was part of the contextual review rather than the clinical systematic review, and did not compare non- opioid treatments to opioids.
The patient voice and preferences were not explicitly included in the guideline. This raises concerns about the patient-centeredness of the guideline.

For far too many years, pain researchers and clinicians have relied on the concept of the morphine equivalent daily dosage (MEDD), or some variant of it, as a means of comparing the "relative corresponding quantity" of the numerous opioid molecules that are important tools in the treatment of chronic pain.
...And, most unfortunately, opioid prescribing guideline committees have relied on this concept as a means of placing (usually arbitrary) limits on the levels of opioids that a physician or other clinician should be allowed to prescribe. Although these guidelines typically bill themselves as "voluntary," their chilling effect on prescribers and adaptation into state laws[2] makes calling them "voluntary" disingenuous.
Although some scientists and clinicians have been questioning the conceptual validity of MEDD for several years, a recent study[3] has indicated that the concept is unequivocally flawed—thereby invalidating its use empirically and as a tool in prescribing guideline development.
The authors used survey data from pharmacists, physicians, nurse practitioners, and physician assistants to estimate daily morphine equivalents and found great inconsistency in their conversions of hydrocodone, fentanyl transdermal patches, methadone, oxycodone, and hydromorphone—illustrating the potential for dramatic underdosing or, in other cases, fatal overdosing.
Patients with chronic pain (particularly that of noncancer origin) who are reliant on opioid analgesia are already sufficiently stigmatized and marginalized[7] to allow this type of practice to continue to be the norm.
Although the use of MEDD in research and, to a greater extent, in practice, is probably due to unawareness of its inaccuracy, we posit that the use of MEDD by recent opioid guideline committees (eg, the Washington State Opioid Guideline Committee[8] and the Centers for Disease Control and Prevention Guideline Committee[9]) in the drafting of their guidelines is based more heavily on disregarding available evidence rather than ignorance. Furthermore, their misconduct in doing so has been more pernicious than the use of MEDD by researchers and individual clinicians, because these guidelines widely affect society as a whole as well as individual patients with persistent pain syndromes. We opine that these committees are strongly dominated by the antiopioid community, whose agenda is to essentially restrict opioid access—irrespective of the lack of data indicating that opioids cannot be a useful tool in the comprehensive treatment of carefully selected and closely monitored patients with chronic pain.

Are Non-Opioid Medications Superior in Treatment of Pain than Opioid Pain Medicine? Ice Cream Flavor Analogy...
In the Oxford University Press, a November 2018 scientific white paper[5] was released that examined the quality of one of the primary studies that have been used to justify the urgent call to drastically reduce opioid pain medication prescribing while claiming that patients are not being harmed in the process.
The study is commonly referred to as ‘the Krebs study’. “The authors concluded that treatment with opioids was not superior to treatment with non opioid medications for improving pain-related function over 12 months.”
Here is an excerpt from the first paragraph of the design section (usually behind a paywall) from the Krebs study that gives the first hint of the bias that led to them to ‘prove’ that opioids were not effective for chronic pain:
“The study was intended to assess long-term outcomes of opioids compared with non opioid medications for chronic pain. The patient selection, though, specifically excluded patients on long-term opioid therapy.” 
Here is an analogy given in the Oxford Journal white paper to illustrate how the study design was compromised:
If I want to do a randomized control study about ice cream flavor preferences (choices being: vanilla, chocolate, or no preference), the results could be manipulated as follows based on these scenarios:
Scenario A: If a study was done that included only current ice-cream consumers, the outcome would certainly be vanilla or chocolate, because of course they have tried it and know which they like.
Scenario B: If a study was done that included all consumers of all food, then it can change the outcome. If the majority of study participants do not even eat ice-cream, than the result would certainly be ‘no preference’. If the majority do eat ice-cream it would likely be ‘chocolate’. Although this study is wider based, it still does not reflect real world findings.
Scenario C: In an even more extreme example, if this same study is conducted excluding anyone who has ever ate ice-cream at all, then the conclusion will again be ‘no preference’ and the entire study/original question becomes so ludicrous that there is no useful information to be extracted from this study and one would logically question why this type of study would even be conducted (although we know the answer to that)
Scenario C above is how the study that has been used to shift the attitudes towards the treatment of pain in our nation's medical community was designed. “One has to look deep into the study to find that they began with 9403 possible patients and excluded 3836 of them just because they had opioids in their EMR. In the JAMA article, they do not state these obvious biases and instead begin the explanation of participants stating they started with 4485 patients and excluded 224 who were opioid or benzo users.” That is the tip of the iceberg to how it is extremely misleading. The Oxford white paper goes into further detail of the studies “many flaws and biases (including the narrow focus on conditions that are historically known to respond poorly to opioid medication management of pain)”, but the study design and participant selection criteria is enough to discredit this entire body of work. Based on study design alone, regardless of what happened next, the result would be that opioids are no more effective than NSAIDs and other non-opioid alternatives.

A Good Man Speaks Truth to Power January 2019
Because I write and speak widely on public health issues and the so-called “opioid crisis”, people frequently send me references to others’ work. One of the more startling articles I’ve seen lately was published November 20, 2018 in Pharmacy Times. It is titled “Should We Believe Patients With Pain?”[9]. The unlikely author is Commander John Burke, “a 40-year veteran of law enforcement, the past president of the National Association of Drug Diversion Investigators, and the president and cofounder of the International Health Facility Diversion Association.”
The last paragraph of Commander Burke’s article is worth repeating here.
“Let’s get back to dealing with each person claiming to be in legitimate pain and believe them until we have solid evidence that they are scamming the system. If they are, then let’s pursue them through vigorous prosecution, but let’s not punish the majority of people receiving opioids who are legitimate patients with pain.”
This seems a remarkable insight from anyone in law enforcement — especially from one who has expressed this view in both Pain News Network, and Dr Lynn Webster’s video “The Painful Truth”. Recognizing Commander Burke’s unique perspective, I followed up by phone to ask several related questions. He has granted permission to publish my paraphrases of his answers here.
“Are there any available source documents which establish widely accepted standards for what comprises “over-prescription?” as viewed by diversion investigators?” Burke’s answer was a resounding “NO”. Each State and Federal Agency that investigates doctors for potentially illegal or inappropriate opioid prescribing is pretty much making up their own standards as they go. Some make reference to the 2016 CDC Guidelines, but others do not.
  1. “Thousands of individual doctors have left pain management practice in recent years due to fears they may be investigated, sanctioned, and lose their licenses if they continue to treat patients with opioid pain relievers.. Are DEA and State authorities really pursuing the worst “bad actors”, or is something else going on?
Burke’s answer: “Regulatory policy varies greatly between jurisdictions. But a hidden factor may be contributing significantly to the aggressiveness of Federal investigators. Federal Agencies may grant financial bonuses to their in-house diversion investigators, based on the volume of fines collected from doctors, nurse practitioners, PAs and others whom they investigate.

"No law enforcement agency at any level should be rewarded with monetary gain and/or promotion due to their work efforts or successes. This practice has always worried me with Federal investigators and is unheard of at the local or state levels of enforcement.”

Commander Burke’s revelation hit me like a thunder-clap. It would explain many of the complaints I have heard from doctors who have been “investigated” or prosecuted. It’s a well known principle that when we subsidize a behavior, we get more of it. Financial rewards to investigators must inevitably foster a “bounty hunter” mentality in some. It seems at least plausible that such bonuses might lead DEA regulators to focus on “low hanging fruit” among doctors who may not be able to defend themselves without being ruined financially. The practice is at the very least unethical. Arguably it can be corrupting.
I also inquired concerning a third issue:
  1. I read complaints from doctors that they have been pursued on trumped-up grounds, coerced and denied appropriate legal defense by confiscation of their assets – which are then added to Agency funds for further actions against other doctors. Investigations are also commonly announced prominently, even before indictments are obtained – a step that seems calculated to destroy the doctor’s practice, regardless of legal outcomes. Some reports indicate that DEA or State authorities have threatened employees with prosecution if they do not confirm improper practices by the doctor. Do you believe such practices are common?”

Burke’s answer: “I hear the same reports you do – and the irony is that such tactics are unnecessary. Lacking an accepted standard for over-prescribing, the gross volume of a doctor’s prescriptions or the dose levels prescribed to their patients can be poor indicators of professional misbehavior. Investigators should instead be looking into the totality of the case, which can include patient reports of poor doctor oversight, overdose-related hospital admissions, and patterns of overdose related deaths that may be linked to a “cocktail” of illicit prescribing. Especially important can be information gleaned from confidential informants – with independent verification – prior patients, and pharmacy information.”

No formal legal prosecution should ever proceed from the testimony of only one witness — even one as well informed as Commander John Burke. But it seems to me that it is high time for the US Senate Judiciary Committee to invite the testimony of others in open public hearings, concerning the practice of possible bounty hunting among Federal investigators.
C50 Patient, Civil Rights Attorney, Maine Department of Health, and Maine Legislature Collaborative Enacted Definition of Palliative Care
One suggestion that our organization would like to make is altering the definition of “palliative care” in such a manner that it can include high-impact or intractable patients; those who are not dying this year, but our lives have been shattered and/or shortened by our diseases and for whom Quality of Life should be the focus. Many of our conditions may not SIGNIFICANTLY shorten my life, therefore I could legitimately be facing 30-40 years of severe pain with little relief; that is no way to live and therefore the concern is a rapidly increasing suicide rate.
This is a definition that one of our coalition members with a civil rights attorney and the Maine Department of Health agreed upon and legislators enacted into statues in Maine. This was in response to a 100mme restriction. This attorney had prepared a lawsuit based on the Americans with Disability Act that the Department of Health in Maine agreed was valid; litigation was never the goal, it was always patient-centered care.
A. "Palliative care" means patient-centered and family-focused medical care that optimizes quality of life by anticipating, preventing and treating suffering caused by a medical illness or a physical injury or condition that substantially affects a patient's quality of life, including, but not limited to, addressing physical, emotional, social and spiritual needs; facilitating patient autonomy and choice of care; providing access to information; discussing the patient's goals for treatment and treatment options, including, when appropriate, hospice care; and managing pain and symptoms comprehensively. Palliative care does not always include a requirement for hospice care or attention to spiritual needs. B. "Serious illness" means a medical illness or physical injury or condition that substantially affects quality of life for more than a short period of time. "Serious illness" includes, but is not limited to, Alzheimer's disease and related dementias, lung disease, cancer, heart, renal or liver failure and chronic, unremitting or intractable pain such as neuropathic pain.

Dear Pharmacists,
The Board of Pharmacy has had an influx of communication concerning patients not able to get controlled substance prescriptions filled for various reasons, even when signs of forgery or fraudulence were not presented. As a result of the increased “refusals to fill,” the board is issuing the following guidance and reminders regarding the practice of pharmacy and dispensing of controlled substances:
  1. Pharmacists must use reasonable knowledge, skill, and professional judgment when evaluating whether to fill a prescription. Extreme caution should be used when deciding not to fill a prescription. A patient who suddenly discontinues a chronic medication may experience negative health consequences;
  2. Part of being a licensed healthcare professional is that you put the patient first. This means that if a pharmacist has any concern regarding a prescription, they should attempt to have a professional conversation with the practitioner to resolve those concerns and not simply refuse the prescription. Being a healthcare professional also means that you use your medication expertise during that dialogue in offering advice on potential alternatives, changes in the prescription strength, directions etc. Simply refusing to fill a prescription without trying to resolve the concern may call into question the knowledge, skill or judgment of the pharmacist and may be deemed unprofessional conduct;
  3. Controlled substance prescriptions are not a “bartering” mechanism. In other words, a pharmacist should not tell a patient that they have refused to fill a prescription and then explain that if they go to a pain specialist to get the same prescription then they will reconsider filling it. Again, this may call into question the knowledge, skill or judgment of the pharmacist;
  4. Yes, there is an opioid crisis. However, this should in no way alter our professional approach to treatment of patients in end-of-life or palliative care situations. Again, the fundamentals of using our professional judgment, skill and knowledge of treatments plays an integral role in who we are as professionals. Refusing to fill prescriptions for these patients without a solid medical reason may call into question whether the pharmacist is informed of current professional practice in the treatment of these medical cases.
  5. If a prescription is refused, there should be sound professional reasons for doing so. Each patient is a unique medical case and should be treated independently as such. Making blanket decisions regarding dispensing of controlled substances may call into question the motivation of the pharmacist and how they are using their knowledge, skill or judgment to best serve the public.
As a professional reminder, failing to practice pharmacy using reasonable knowledge, skill, competence, and safety for the public may result in disciplinary actions under Alaska statute and regulation. These laws are:
AS 08.80.261 DISCIPLINARY ACTIONS
(a)The board may deny a license to an applicant or, after a hearing, impose a disciplinary sanction authorized under AS 08.01.075 on a person licensed under this chapter when the board finds that the applicant or licensee, as applicable, …
(7) is incapable of engaging in the practice of pharmacy with reasonable skill, competence, and safety for the public because of
(A) professional incompetence; (B) failure to keep informed of or use current professional theories or practices; or (E) other factors determined by the board;
(14) engaged in unprofessional conduct, as defined in regulations of the board.

(15) failing to use reasonable knowledge, skills, or judgment in the practice of pharmacy;
(b) The board will, in its discretion, revoke a license if the licensee …
(4) intentionally or negligently engages in conduct that results in a significant risk to the health or safety of a patient or injury to a patient; (5) is professionally incompetent if the incompetence results in a significant risk of injury to a patient.
(c) The board will, in its discretion, suspend a license for up to two years followed by probation of not less than two years if the licensee ...
(2) is professionally incompetent if the incompetence results in the public health, safety, or welfare being placed at risk.
We all acknowledge that Alaska is in the midst of an opioid crisis. While there are published guidelines and literature to assist all healthcare professionals in up to date approaches and recommendations for medical treatments per diagnosis, do not confuse guidelines with law; they are not the same thing.
Pharmacists have an obligation and responsibility under Title 21 Code of Federal Regulations 1306.04(a), and a pharmacist may use professional judgment to refuse filling a prescription. However, how an individual pharmacist approaches that particular situation is unique and can be complex. The Board of Pharmacy does not recommend refusing prescriptions without first trying to resolve your concerns with the prescribing practitioner as the primary member of the healthcare team. Patients may also serve as a basic source of information to understand some aspects of their treatment; do not rule them out in your dialogue.
If in doubt, we always recommend partnering with the prescribing practitioner. We are all licensed healthcare professionals and have a duty to use our knowledge, skill, and judgment to improve patient outcomes and keep them safe.

“The opioid crisis has had a tragic impact on individuals, families, and communities throughout the country. We’re in urgent need of new and better treatment options for opioid use disorder. The guidance we’re finalizing today is one of the many steps we’re taking to help advance the development of new treatments for opioid use disorder, and promote novel formulations or delivery mechanisms of existing drugs to better tailor available medicines to individuals’ needs,” said FDA Commissioner Scott Gottlieb, M.D. “Our goal is to advance the development of new and better ways of treating opioid use disorder to help more Americans access successful treatments. Unfortunately, far too few people who are addicted to opioids are offered an adequate chance for treatment that uses medications. In part, this is because private insurance coverage for treatment with medications is often inadequate. Even among those who can access some sort of treatment, it’s often prohibitively difficult to access FDA-approved addiction medications. While states are adopting better coverage owing to new legislation and resources, among public insurance plans there are still a number of states that are not covering all three FDA-approved addiction medications. To support more widespread adoption of medication-assisted treatment, the FDA will also continue to take steps to address the unfortunate stigma that’s sometimes associated with use of these products. It’s part of the FDA’s public health mandate to promote appropriate use of therapies.
Misunderstanding around these products, even among some in the medical and addiction fields, enables stigma to attach to their use. These views can serve to keep patients who are seeking treatment from reaching their goal. That stigma reflects a perspective some have that a patient is still suffering from addiction even when they’re in full recovery, just because they require medication to treat their illness. This owes to a key misunderstanding of the difference between a physical dependence and an addiction. Because of the biology of the human body, everyone who uses a meaningful dose of opioids for a modest length of time develops a physical dependence. This means that there are withdrawal symptoms after the use stops.
A physical dependence to an opioid drug is very different than being addicted to such a medication. Addiction requires the continued use of opioids despite harmful consequences on someone’s life. Addiction involves a psychological preoccupation to obtain and use opioids above and beyond a physical dependence.
But someone who is physically dependent on opioids as a result of the treatment of pain but who is not craving the drugs is not addicted.
The same principle applies to replacement therapy used to treat opioid addiction. Someone who requires long-term treatment for opioid addiction with medications, including those that are partial or complete opioid agonists and can create a physical dependence, isn’t addicted to those medications. With the right treatments coupled to psychosocial support, recovery from opioid addiction is possible. The FDA remains committed to using all of our tools and authorities to help those currently addicted to opioids, while taking steps to prevent new cases of addiction.”

Maryland’s co-prescribing new laws/ amendments regarding benzos and opioids
Chapter 215 AN ACT concerning Health Care Providers – Opioid and Benzodiazepine Prescriptions – Discussion of Information Benefits and Risks
FOR the purpose of requiring that certain patients be advised of the benefits and risks associated with the prescription of certain opioids, and benzodiazepines under certain circumstances, providing that a violation of this Act is grounds for disciplinary action by a certain health occupations board; and generally relating to advice regarding benefits and risks associated with opioids and benzodiazepines that are controlled dangerous substances.
Section 1–223 Article – Health Occupations Section 4–315(a)(35), 8–316(a)(36), 14–404(a)(43), and 16–311(a)(8) SECTION 1. BE IT ENACTED BY THE GENERAL ASSEMBLY OF MARYLAND, That the Laws of Maryland read as follows: Article – Health Occupations (a) In this section, “controlled dangerous substance” has the meaning stated in § 5–101 of the Criminal Law Article.
Ch. 215 2018 LAWS OF MARYLAND (B) On treatment for pain, a health care provider, based on the clinical judgment of the health care provider, shall prescribe: (1) The lowest effective dose of an opioid; and (2)A quantity that is no greater than the quantity needed for the expected duration of pain severe enough to require an opioid that is a controlled dangerous substance unless the opioid is prescribed to treat: (a.) A substance–related disorder; (b.) Pain associated with a cancer diagnosis; (c.) Pain experienced while the patient is receiving end–of–life, hospice, or palliative care services; or (d.) Chronic pain
(C.) The dosage, quantity, and duration of an opioid prescribed under [subsection (b)] of this [section] shall be based on an evidence–based clinical guideline for prescribing controlled dangerous substances that is appropriate for: (1.) The health care service delivery setting for the patient; (2.) The type of health care services required by the patient; (3.) and The age and health status of the patient.
(D) (1) WHEN A PATIENT IS PRESCRIBED AN OPIOID UNDER SUBSECTION (B) OF THIS SECTION, THE PATIENT SHALL BE ADVISED OF THE BENEFITS AND RISKS ASSOCIATED WITH THE OPIOID.
 (2) WHEN A PATIENT IS CO–PRESCRIBED A BENZODIAZEPINE WITH AN OPIOID THAT IS PRESCRIBED UNDER SUBSECTION (B) OF THIS SECTION, THE PATIENT SHALL BE ADVISED OF THE BENEFITS AND RISKS ASSOCIATED WITH THE BENZODIAZEPINE AND THE CO–PRESCRIPTION OF THE BENZODIAZEPINE. 
(E) A violation of [subsection (b) OR (D) of] this section is grounds for disciplinary action by the health occupations board that regulates the health care provider who commits the violation.
4-315 (a) Subject to the hearing provisions of § 4–318 of this subtitle, the Board may deny a general license to practice dentistry, a limited license to practice dentistry, or a teacher’s license to practice dentistry to any applicant, reprimand any licensed dentist, place any licensed dentist on probation, or suspend or revoke the license of any licensed dentist, if the applicant or licensee: (35) Fails to comply with § 1–223 of this article.
8–316. (a) Subject to the hearing provisions of § 8–317 of this subtitle, the Board may deny a license or grant a license, including a license subject to a reprimand, probation, or suspension, to any applicant, reprimand any licensee, place any licensee on probation, or suspend or revoke the license of a licensee if the applicant or licensee: (36) Fails to comply with § 1–223 of this article.
14–404. (a) Subject to the hearing provisions of § 14–405 of this subtitle, a disciplinary panel, on the affirmative vote of a majority of the quorum of the disciplinary panel, may reprimand any licensee, place any licensee on probation, or suspend or revoke a license if the licensee: (43) Fails to comply with § 1–223 of this article.
16–311. (a) Subject to the hearing provisions of § 16–313 of this subtitle, the Board, on the affirmative vote of a majority of its members then serving, may deny a license or a limited license to any applicant, reprimand any licensee or holder of a limited license, impose an administrative monetary penalty not exceeding $50,000 on any licensee or holder of a limited license, place any licensee or holder of a limited license on probation, or suspend or revoke a license or a limited license if the applicant, licensee, or holder:
(8) Prescribes or distributes a controlled dangerous substance to any other person in violation of the law, including in violation of § 1–223 of this article;
SECTION 2. AND BE IT FURTHER ENACTED, That this Act shall take effect October 1, 2018.

I met Leftenant Aleksandr Merriam only once off of the battlefield. They say first impressions matter, and my first impression was this was a young man who wouldn't need to worry about the long term health impacts of eating, drinking and smoking at high-class soirees every evening.
The reason? Arrogance.
We met in the 'Mech bay. His Lance was short and me being something of a free agent, I was hired to plug the gap. His family 'Mech, a Chameleon of all things, was pristine; the blue, white and red of his regiment worn so proudly not even a single speck of dust could be suffered upon its surface.
Next to him was my own machine; a Shadow Hawk. On paper it was a sunset amber with a single diagonal stripe of white. I say "on paper" because life had never been kind to me or my 'Mech; orange-brown dust and dirt smeared the chassis, blackened wounds of old laser burns sliced across armour plating; the entire left leg was stolen from a wrecked machine and still bore the red livery of the former owner. Time and the elements had done a number on the rest, washing out the vibrancy of the colours so that the 'white' stripe was now more light grey. Bare metal showed where it shouldn't. It could all have been fixed, I suppose, but after a while I'd realised there wasn't much point in caring about such things. Besides, the heraldry my machine wore was of a mercenary formation now disbanded.
Still, I wasn't there to look pretty. I was there to kill Capellans. Not so with the Leftenant. He made sure to voice his displeasure at the shabby nature of my machine, as if the repulsed sneer upon his face wasn't obvious enough. He even went so far as to refuse to let me drop unless I 'fixed' it. I told him "as long as your men do the work and foot the bill, you can paint my 'Mech however you like."
By the time we dropped, I was sporting those shiny Davion colours too. I could still smell the wet paint scent from within the cockpit.
While my temporary commanding officer was deeply concerned with looking the part, I was concerned with getting paid and living to spend said pay. That's why I wasn't keen on boldly marching straight towards the enemy's last known position without so much as a Locust running recon. If it were up to me, we'd have held position in a defensible location and the Leftenant could have put that breeding to good use; scrounge up some reinforcements, that sort of thing. There was one Capellan Lance we knew about, so ideally I wanted us to hit them with three of our own.
Ah, but there's no "glory" in that. Not according to Aleksandr. So we were going in head on, equal numbers, as God intended.
We met the Capellans on fairly open ground. When they started to pull back from said skirmish I strongly urged the Lance to hold rather than pursue. It's not like the OpFor had started running as soon as our guns barked, you understand; they'd slugged it out with the desperate fury of men whose backs were against the wall. Our Thunderbolt had taken such a beating most of his torso armour was gone and the right arm lay smoking on the ground. My own machine had a dozen black scars slicing across my left side, as if the white and red bars of House Davion's heraldry offended the Capellans so much they wanted to burn it off my 'Mech.
My gut, which was a well-informed gut that knew far more about being a MechWarrior than most men's brains, was screaming "trap" as the Capellans pulled back. I managed to persuade two of my Lancemates. Guess which one wasn't sold.
Last I heard of Aleksandr Merriam was him screaming at us, calling us cowards, threatening martial punishments and following up with every inventive slur under the sun as he barrelled towards what he felt certain was a Capellan force on the brink of defeat. Last I saw of him was a blinding flash as he strode valiantly into a minefield.
But hey, at least he looked fantastic doing it.
